Our 100 year old, and very “with it”, mother is in Berlin Rehab following emergency surgery in June. She is doing great and they have been treating her like a Queen! PT has her walking 90 feet with a walker and we know in time she will be back to where she was before the surgery. She likes the food and all the attention she’s getting! Would definitely recommend this Rehab!!!

DISGRACEFUL-Do not trust your family member to be safe at Berlin Rehab. My father was admitted on 2/27 and had to be in isolation due to coming in with covid. After having a meeting with the social worker Gina on 3/1 we were told my father would be moved out of isolation on 3/3 as his 10 days of required quarantine would be finished. About 2 hours after my meeting my father was told they were moving him today 3/1 to another room. When my father arrived to the new room there was no bed available in the room for him and he was told by the aide they didn't realize the bed was missing and they had to go find one and left him sitting in room in a wheel chair. Mind you they moved him to a floor where only longterm care patients are not rehab patients. On top of this the roommate he was placed with has an extreme mental/psychosis illness and started screaming and cursing at my father as soon as he arrived to room. After being advised of this travesty I made some contacts to the rehab and spoke to the business manger who assured me she knew the man and he is nice. I drove straight to the rehab to find my dad in distress from his roommates behavior. The man was balled up in fetal position in the bed and continuously was screaming he wanted to die and kill himself, he was screaming to stop talking and many other disturbing statements. I walked out of the room to discuss with the nurses and aides and was told by multiple employee's on that floor that this patient screams like this all day and night and any roommates of the past have had the same issues and needed to be moved. I did not feel my father was safe being left in this room with a patient who obviously had mental status concerns. They also had to keep the door shut at all times because of the covid regulations as both of them were considered covid positive. My father's nurse call bell in this room was also not working which they said they would address and did not. I requested to speak with the night supervisor who's name is Karen and after explaining all my concerns to her she showed no compassion and just advised there was nothing she could do there was no other place for my father. I told her if they didn't find another room for him tonight I was taking him home with me. Now my father is 76 years old recovering from Covid and has lost most of his mobility from his waist down and has extreme weakness all over and needs to have this PT done to function. I would do anything at this point to make sure my father was in a safe place where he would not have to feel uncomfortable and have feared what could happen since the door was closed at all times and the lights go out. Karen allowed me to remove my father without any other options and just said I would be required to sign forms saying I was removing him against best medical judgement. How about medical safety which this place obviously knows nothing or cares about. THIS PLACE IS A DISGRACE AND DOES NOT CARE ABOUT THE PATIENT!!!!!

I do have to mention and give a shout out as horrible as the experience was, the aide Sierra on the the second floor was amazing and went above and beyond to help me get my father ready to take him out of this non caring facility. Thank you Sierra for all your help...you are a...

Major changes since the change in ownership. My mum has been there for almost three years. Almost all agency nurses so there is little chance for the aides to get to know the residents. Most regular aides who knew the residents well left (many with 10 plus years experience) because the new owners offered poor health care packages, which was corroborated by the aides who were leaving and by a VP at Marquis who told me that poor health care coverage was the reason people we’re leaving, but that Marquis was unable to offer any other coverage. The administrator (who had been there for 12 years) the DON, the Activities Director, and the facility’s doctor (also 12 years) all resigned within the same 2 week period. To be fair, I must say that the doctor who has taken over my mum’s care has been wonderful about reaching out and keeping in touch.

The food service doesn’t follow the printed menus that they pass out. My mum fills out her menu, and the kitchen sends up completely different meals. For most of the last month my mum has only received 3 diapers a day, even after multiple phone calls, so my brother and I have had to purchase supplemental supplies.

I complained repeatedly to Kay Weiss (a corporate VP who gave me her cell phone number) and she was responsive the first couple of times I called, but she still hasn’t returned my phone calls from four weeks ago regarding my mum’s lack of diapers and discrepancies with my mum’s patient discretionary account. I called the number listed on the bill (Shira Cohen) but I received a phone call from the prior administrator telling me I had to deal directly with the facility, but the former DON didn’t return my phone calls, and the person who I had always addressed financial questions no longer had that job, so I could only reach out via phone and email to the contact I was given. I eventually tracked down someone in the facility, but it still isn’t straightened out. I also received bills from my mother’s supplemental insurance company that the facility either hadn’t paid or else paid her policy late, this generating slate payment notice, that went to my mum, who doesn’t pay any bills. When I called to complain, I was just told that shouldn’t have happened and they’ll need to look into it.

I continue to be hopeful that things will improve My mum says that the aides who care for her are good and she is fine because she can speak up for herself, but she worries for other residents who can’t advocate for themselves.

Re the new administration, I’ve received no letters or emails introducing the new administrator and the new DON, but hopefully they’ll reach out soon. The nurse on my mum’s unit has been there for the entire time my mum has been there and I know she’s looking out for my mum because she reaches out to...

I contemplated on writing this review because I did interact with two amazing nurses at this facility. However, I would feel extremely horrible as a human being to not write this review and let you know the lack of attention your elderly will receive. My mother went for rehabilitation after suffering from two small strokes initially they put her on the first floor with no bed alarm or chair alarm, when she has been deemed a high fall risk because she has a history of previous strokes already. While there were a few CNA’s that were very nice, we were met by a male nurse who had the worst attitude and seemed bother by anything we asked for. After a few weeks on the first floor, they sent her up to the second floor where their assisted living unit is. You have patients that have major health issues, such as dementia, Alzheimer’s, bed ridden, etc. I didn’t oppose because I figured my mother needed more attention and I guess this is why they put her in this unit although she was only there for the rehab portion of their establishment. To begin this place is not conducive for the Latino community since they have no Spanish channels on their TV that is very outdated and small. Secondly, you would think that mom being a fall risk on the first floor ( where she She had four falls in a week) she would have gotten the proper attention. I did complain because this is a rehabilitation center where she should be getting better and not worse. It’s counterproductive. to continue, she falls face first off of her bed, once again, there were no better alarms. I get a phone call at 7:30 in the morning, letting me know about this hard fall and then proceeds to tell me that she was thoroughly checked and that everything was OK and did not need to go to the hospital. The next day they discharge her back to her assisted-living. Today my mother is in the hospital just five days later with a massive blood clot over her left eye where you see that lump. She also has brain bleeding and has to go into emergency surgery. They have placed her in the ICU. as her daughter, I am her advocate, her voice, and I am completely devastated by the outcome of this place. I initially chose this place. They had a decent rating. I wouldn’t recommend it to anyone. It is cold. The higher-ups are cold. you feel like you are bothering everyone for Everything. Take your elders elsewhere. These pictures were taken on the day of the fall.

Update: due to a Subdural Hematoma (from the fall) mom had to endure brain surgery and ultimately…… I can’t...
