I find only one word fitting for a place like this… neglectful. Understaff is putting the term nicely. I saw two nurses on the entire floor. Support staff is unfriendly and lacks compassion. There is a women crying in the hallway that she soiled herself. Regardless if it was true or not, how do you walk pass someone who is in distress and not try to comfort them. absolutely horrible Update 6/21… I wish I could report that things improved. Unfortunately no! My mother went to visit my grandmother today. She arrived after 3pm. My grandmother was found wearing the same pajamas she was wearing the day prior with a diaper that was saturated with urine and feces. My mother asked an aide for a diaper and was told they only have size small. This place is a train wreck. The other day the nursing supervisor asked my grandma her name. (Reminder she has dementia) She provided the nurse with her maidan name. We were told by the supervisor that my grandmother tried to trick her by giving her a different name. Sometimes patients with dementia are not oriented to even whom they are. This should be know to a nursing supervisor in a SNF. I am at a loss for words. Close the unit if you don’t have enough staff. Patient safety should be more important than making a profit. Below is the milk she was served with her dinner on 6/21. Expired milk absolutely disgusting! My mother met with the speech therapist and told she won’t be the first or the last person to leave the facility unhappy.

My family and I are very thankful for the wonderful treatment our mother is receiving at Luxor, Mills Pond. The staff is kind and warm. They are doing everything they can to get mom back on her feet after 3 weeks in a bed. She is 89 years old. A very strong and vibrant woman. She was in sepsis at the hospital, and released to Luxor while still very ill. I have lost faith in Stony Brook University hospital. She couldn't stand, eat, or move. She had a catheter and a diaper. Her hemoglobin was at 8.4. Heart rate of 50. Luxor has taken her off medications that were causing the extremely low BP and a dietitian came to her room and customized a food plan specifically for her . After less than 2 weeks, she is getting out of bed and attending physical therapy. She is eating and they are keeping her well hydrated. No more catheter or diaper! This rehab is special! I am so grateful that she is being so well cared for there. Thank you to all of the wonderful staff!

Staff was very welcoming and friendly. The atmosphere was positive and happy with the bright colorful pictures displayed all over the hallways. My friend is being well taken care of, and he says the staff is nice to him. He introduced me to them by name as they entered his room to do their jobs. Very positive/nice experience I had during my first time visit there.

Staff was very welcoming and friendly. The atmosphere was positive and happy with the bright colorful pictures displayed all over the hallways. My friend is being well taken care of, and he says the staff is nice to him. He introduced me to them by name as they entered his room to do their jobs. Very positive/nice experience I had during my first time visit there.

I am pleased to be able to write this review in support of the quality and outstanding care I received when admitted to the Luxor Rehabilitation and Nursing care Center at Mills Pond in St. James New York. After being in a hospital for several weeks, being treated for an autoimmune disease, unrelated to COVID 19, once stabilized I was transferred to the rehabilitation facility. This would allow me to improve an increased level of independence so that I would be able to go into an Assisted Living center or to go back to my home.

I, my wife, and family, as I am sure many of you would, had a great deal of anxiety regarding thre adjustment and quality of care I would receive at a rehabilitation and long-term nursing care center. Let me state at the ourtset, without question, my choice to stay at the Luxor Rehabilitation and Nursing Center was the right choice. From the time I was admitted to the day of my discharge every single individual, everyday, gave me excellent and quality care which during my six month stay reduced my anxiety to zero. You could say that the care given to me was like a well oiled machine, or a pie where every piece fit together perfectly. There was great communication between all departments which assured that the care I received was seamless.

I am pleased to say that the level of care I received was far beyond what I expected from a rehabilitation and nursing home. There is an old adage that states, "It is not the job that one does but it is the person that does that job". I wish I could name all of the outstanding caregivers but here are just a few who deserve special recognition: The Housekeeping staff, Social Worker department, Alexa in admissions, Kathy in finance, Kathy in Nursing, Michael in administration, Louis, Mary, AnneMarie, and Meghan, in Nursing, and especially Yoleen, Shirley, Ansley, and Darlyne, the best Certified Nursing Assistants I could have hoped to have taking care of me.

Should anyone need Long Term Rehabilitation or Nursing Care, The Luxor Rehabilitation and Nursing Care Center at Mills Pond in St. James New York is the place to be.

I can assure, without question, that those who are there will receive excellent quality care far above and beyond what is expected and which is given to all patients and residents, They will receive empathy, concern, patience, and heartfelt caring. The outstanding staff made a choice to pursue a career path that requires passion, compassion, patience, and discipline, which is currently exacerbated by COVID 19. Most people have an underlying fear of being dependent on someone else for their basic day to day functions, specially with strangers. When it comes to rehab my family has had so much experience with Luxor Mills Pond St. James as our mom has been admitted there too many times to remember. Each time hospitals spoke to us about where to send mom for rehab we only had one response - Luxor. Mom had, however, been in a few others close bye in the past and they were never our choice to return to. She had been on the first floor both sides and short term on the 2nd floor then since February moved to rehab on the 3rd floor. It didn't matter where she was all staff knew her and remembered her. The hospitals all kept thinking mom had pneumonia and it was brought to our attention from Meghan from Speech that she felt mom didn't have pneumonia but rather a swallowing defect and had a lung aspiration from swallowing thin liquids. So my sister at Meghan's suggestion took my mom for a swallow study and sure enough she was right. She worked endless hours with mom teaching her how to swallow and altered drinks and food. We will forever be grateful to her. This last January mom had a horrible aspiration from home and was rushed to the hospital then admitted back to Luxor for rehab. She was admitted to the 3rd floor. It didn't matter what floor all the therapists knew mom and we're all amazing. It was at this time that we made the medical decision to admit mom to long term care there. We cannot commend enough to Sam the nurse manager on the 3rd floor. She was and is always up to date about mom and everything having to do with mom. Always returning calls and on top of everything. We truly are blessed to have charge nurses that are the most amazing at their jobs and should be compensated accordingly. They truly love mom and it shows on every shift. Dawn, Captain and Sheryl. I truly don't know how Dawn does it. Beyond gifted as a nurse. You need anything, she's there before you can ask. The love of her patients shows bright in every long shift. Sheryl is our night owl. Always on top of anything you ask for with a smile and also loves Mom. A wonderful and talented nurse. Captain is always around after his schooling and you would never know he wasn't on the floor for days but he knows everything and attends to every need with a smile. He's amazing. God bless them all. There are several other nurses but to name them all is hard but Sharon and Doreen are wonderful. There are some aides that are good at what they do but Gurleen goes above and beyond on all her shifts. She knows mom and helps and treats her with so much love. Don't hesitate to place a loved one on the 3rd floor. Thank you all for giving my family peace of mind knowing that you love and care for mom. Happy Thanksgiving and we are thankful...

I am mind blown at the lack of attentiveness. My father just got there, I have tried calling the facility over 20 times to speak to nursing staff, supervision, or literally ANYONE that has any sort of knowledge or authority about patient care and no one would answer the phone. I was transferred, hung up on and disregarded multiple times. I voiced my concerns about his care upon his arrival to MULTIPLE people who all assured me that he would be looked after. I have no knowledge on anything at the moment and I am concerned on if the staff is even awake. The care should not be neglected because it’s the evening. I need a simple update and I can’t get a response. I would take him home or to the hospital if it wasn’t for the fact that he hasn’t been stable anywhere in a week and I was hoping and praying he would find some stability here temporarily while the “staff” is working on getting him stronger to come home and I don’t even know if he’s awake/alive at the moment. Disgusting how the staff acts at night and I am terrified that the day time staff is no better.

Update: I wrote this review in the evening when I wasn’t getting a response obviously. Fast forward 2 hours to when I finally got a call that my father had been transferred to the ER because he became unresponsive. I am not angry for this place getting him better care/help and sending him to the hospital. I am angry because I feel it could have all been avoided had they listened to the loved one, myself, about the multiple warnings I tried to give and their lack of ability to get his diabetes under control. There’s much more to go into this story but let’s be realistic, I won’t be heard, this will get shoved under the rug and someone is going to be severely harmed because of situations like this. I was hung up on because I was being “inappropriate” according to the staff member after expressing confusion, anger, anxiety, sadness and concern. Being hung up on was the cherry on top to the staff not answering the phone for hours. I’m sure a lot of people have heard horror stories about nursing homes/rehabilitation centers for multiple reasons but there needs to be a significant amount of awareness raised to the rudeness of staff and lack of attentiveness. I should have prefaced this by saying my father looked absolutely horrible/miserable when I first saw him in this facility and he looked significantly better once I was there to raise awareness to him. We can’t stay there 24/7 but it seems like that’s the only thing that gets people to pay attention. STAY AWAY...
