Rolls n more — Restaurant in Delhi

Ordered paneer tikka roll from here and gave special instructions of not to add onions in it.

As you can see in the images it was loaded with onions and moreover as you can see in the other images., I suspect that there is egg because for sure that's not paneer.

How can we expect these blunders? Who is responsible for this? The staff/the chef should atleast take care when serving vegetarian food that the pan should be different/cleaned before cooking vegetarian food.
