We arrived at the Ocean Cay Pier by a MSC cruise ship. This destination is unlike most other destinations in that booking beforehand is essentially the same price as booking while on shore. Unlike other destinations where you can negotiate with the locals, this is a private island and that's not the case.
There are several things to do on the island, the most common is to relax on the beach. There are two different sides of the beach one of which is very Rocky and the other is more Sandy for lounging and getting into the water. There are strategically placed "Deep Water" to let you know that there is a sudden drop off. One side, the not Rocky side, has a lifeguard, but there's only like one or two for the whole beach side so, be careful. There's everything available there from kayaking to jet skiing and wind sailing and more. You just have to find the post.
As soon as you get off of the boat and walk on to Ocean Cay here there are several shops at which you can purchase MSC or Ocean Cay branded trinkets. The island is only open from 9:00 a.m. until 11:30 p.m., so if you are not back on your boat you'll just be locked out for the night. The cruise provided some free options for food, but they close at 5:00 p.m., and the options are few. Basically, the food that was included with a cruise package included Burgers hot dogs fries and ice cream. If your plant-based, there next to no options for you, aside from fries, maybe pasta.

We visited in August 2024 and it is still undergoing development. What has been done so far looks nice. Very peaceful and laid back theme (island life).
The lighthouse was closed when we visited but at night there was still the light show. It looked nice but nothing too spectacular.
There is a dock so no need to tender. Wheelchair and mobile assistance were easy to obtain or utilize.
There are paved areas . And for sand there are specially provided wheelchair as well. I imagine you have to register with msc for wheelchair assistance to use. As we did (we brought our own wheelchair).
It was raining heavily when we got off and the bars etc were closed. This didn't bother us as we enjoyed the peaceful vibe and moved around when the rain stopped.
After a while it became more crowded as more people then came off the ship.
Overall it has a nice peaceful vibe. Can be crowded and people tend to go to the nearer facilities. There are other areas to explore if you have the time.
As with MSC chairs etc are value added unless built into a package. If you take out a towel make sure to take it back with you inside unless you are yacht club or have a package with...

We initially went to the South Beach lagoon area. My wife and I along with other people were complaining about something in the water irritating them. They thought it was sea lice but we did not see anywhere where what sea lice is. We decided to go over and try the north side beach to see if the waves affected the sea lice or whatever was in the water.
You drink package or Siri and they give you food on the island for free. They've got a lot of work to do. Seems like the only thing to do on the island is go to the beach snorkel and jet ski.
It'd be nice if they offered a swimming pools on the island.
We've been too many other private islands on other cruise lines and this is probably the worst experience.
