I recently had a CVA and as an employee of Life Care of Centerville I chose to go there for my rehab knowing the Therapists personally made that decision easy. I would like to thank the DOR Kim Span for my personal plan of care. Jamie Banes, Diane Pinkerton in ST and Melody in OT who made sure I got a shower as requested. The nursing department was amazing in their care. Lori Qualls waited for my arrival to make certain everything was correct. Wendy Dill had to tolerate me after a long ambulance ride from Nashville. I don’t remember a lot of my time there but I do remember the kindness and compassion shown to me by everyone. Shelby Warren, Wendy CC, Faith from night shift, Karen Davidson, Marsha Sanders, Tylana Gay, Tosha Grimes, Linda Turner, Tracy McCord, Haylea Patterson, Tina Barber, BJ Shanes, Jessica Conner, Christine Whitaker and of course Emily!! Kim Breece and Miko from Activities and many more I can’t think of right now. When I do I will edit my review. I would like to also thank our ED Beverly Wall for everything she did to make certain my stay was pleasant. I had 2 major disappointments. Both occurring on the morning of my discharge. Each and every resident is to be discharged with a 30 day supply of medication. That morning I knew something was wrong and Lori Qualls tried to explain it to me but I was too confused to understand. This should have have been explained to my husband when he came to pick me up. A medication I was to take twice a day, I was discharged with only one tablet. Leaving a confused resident and her 77 y/o husband to find out that day while they were unpacking her belongings and looking at the paperwork. (I should mention that my stroke activated seizures that lasted for days landing me in ICU on a drip to keep me sedated. This medication I was discharged without was an anti seizure med) After making numerous phone calls to Skyline Neurology without success, we reached out to my PCP, Tammy Milholen NP, who was kind enough to call a 30 day supply in to our local pharmacy. This is a situation that should have certainly been handled by Life Care of Centerville. If not prior to my discharge then by the end of the business day. The other disappointment I had was upon discharge as well. We call a Code H when any resident goes home. All available staff are supposed to attend and cheer the leaving resident on as they head for home. The DON, Brandi Niece, chose to continue her morning rounds instead of coming to tell me goodbye, thanking me for choosing her facility and meeting my husband. It certainly was not because she was not aware of the situation either as several employees told her I was waiting for her at the front door. This occurrence has stayed with me since I have been home and truly made me realize how important I was to her as almost a 5 year employee (5 years in January) who chose her facility. Regardless my stay was pleasant. The housekeeping staff was amazing. Mine was Amy Weems and Lisa Pellerin, they both did an excellent job and spent a few extra minutes with me as well. Mrs Willie tried her best to get me to eat, sometimes I would, sometimes I wouldn’t. One night I ate 3 suppers. The food was sufficient and the dietary staff went out of their way to make sure I had everything I requested and more. If you are in need of short or long term care please don’t avoid choosing Life Care of Centerville because of our lack of updates. Lack of updates do not equal lack of care. Even though I think our building reflects that and other places get chosen due to their amenities. I would like to take this time to ask Corporate to spend some money on these much needed updates as I truly believe this alone would benefit our Census, residents and staff. It was the cleanest best maintained nursing home I have ever been in. Usually entering a nursing home your a tad bit older than me but being able to sit back and watch this staff in action was amazing.
