Tennessee EMS Run Report Requirements (Rule 1200-12-01-.15)

Run reports must be submitted to the Division of EMS within 60 days of the incident.

If a complete report cannot be left with the receiving hospital immediately, it must be submitted within 24 hours of transport.

Serious incidents (serious injury related to EMS care, equipment failures, etc.) must be reported to the Division within 5 business days of discovery.

Expectations for Clay County EMS

Complete run reports immediately after each call, preferably before returning to service.

Submit documentation in advance of the 60-day deadline—late reports are not acceptable.

Provide a written/electronic report to hospitals within 24 hours if it cannot be provided at the time of transport.

Report any serious EMS-related injury or equipment failure immediately to supervision; ensure Division notification is completed within 5 business days.

State Enforcement Actions for Non-Compliance

The Tennessee Department of Health / EMS Division treats documentation compliance as a licensure requirement. If an EMS provider or service fails to meet these deadlines, the following actions may be taken:

Written deficiencies or corrective action orders issued to the provider/service.

Civil penalties or fines imposed by the Board of EMS.

Service license probation or suspension for repeated failures.

Individual provider discipline, including suspension or revocation of an EMS license, for ongoing or willful non-compliance.

Withholding of service license renewal until deficiencies are corrected.
