My parents moved into Assisted Living when this building first opened. There were so many selling points that we thought it would be a great place. Betty is a great saleswomen. The intention was for my parents to live there and never have to move. Sadly my parents health declined after moving in and both of them had losses in mobility. Two years later we are sad to say that my father died of a heart attack while living there less than a year and my mother is no longer welcome there. After falling and breaking her hip and being a patient in their skilled nursing facility for 6 weeks the administrator told us she no longer qualified for assisted living or any other Oak Park Place residency. This was disappointing to my mother since she had planned to live there the rest of her life. I however was ready for her to move anyway because of the struggles we had there to get the appropriate support. Our biggest complaints seem to be consistent with other reviews. Many great staff but never enough staff to meet resident needs and there was A LOT of turnover. They have gone through 4 different administrators and 3 head nurses...so consistency has been a problem. Our biggest grievances include: 1. Not responding with concern in an emergency situation in assisted living 2. No social interaction, exercise, church, activities or common meals for 8 months + in assisted living because of COVID 3. Not leaving patient who requires a 1 person assist with the ability to call for help in skilled nursing (Call light not with patient multiple times) 4. Facility is not able to share much data about patient needs with family/activated HCPOA. 5. For over a year administrative staff did not respond to multiple requests for patient to move apartments to be closer to nurses station or to hold a care team meeting. (a meeting finally occurred but no solution was ever found) 6. Formal complaints made with both the on site sales director and regional director with no follow up. It was clear when my parents moved in that my mother would need an increase in services over time given her Parkinson's disease but when the increase was needed my mother's needs were not able to be met. My best friends dad had just passed this past weekend, she had a camera in the room because of previous incidents and neglect that have taken place. Her father had been given his food and on his diet was to be purred. There is a high probability he was given regular food due to the fact of someone not checking and on the camera he was chocking and he was allowed to drowned in his own vomit and mucus. The video shows that the nurse Vikki the nurse said “he always does this” this is and walked away. I was in the room and witnessed his passing and the Negligent staff who didn’t even come in to see if he was comfortable during this time, no was there a nurse on staff or a stethoscope on the unit for “Me “ to confirm his passing… yes say that out loud it’s God awful, this place has multiple complaints. The DON Director of Nursing is never available nor present on the units. They don’t even have CNA or CMA on the unit they are trained in 2 days and are not certified and are patient care assistants. As I was interviewing them during my time there. We will never get my friend’s father back, but we are trying to bring awareness of neglect and lack of accountability! The outside is beautiful the staff is incompetent and the Director of Nursing should be ashamed of herself to allow a facility like this to be ran under her name is Leslie. The state has been in on several occasions and nothing happens so pls be aware and read . This man had aspirated his food and all could have been avoided we have time lines on the cameras that have shown no one was in his room for 12 hours at a time . Time for change! We don’t want this to happen to any other families! The long term staff was amazing, however all the new employees were not trained . Read moreDo not bring your loved ones here. Horribly understaffed. Calls went unanswered. He once went THREE WEEKS without a shower. On a floor with 20 residents there were only 2-3 nurses whenever I visited. The bathroom was rarely cleaned. A nurse stole at least $200 in cash from my grandfather's wallet. Also stolen was his only phone charger and since his phone was dead he couldn't tell me about it until I visited him. Talked to admin about it and they blamed HIM for keeping cash and valuables in his room. No investigation occurred, and the suspected thief quit shortly after the theft. He chose Oak Park Place because he could bring his cat, but when his health declined rapidly (after contracting COVID here in his private room) and needed assistance feeding his cat this place deemed him unworthy of having a companion. So, a higher up TOOK HIS CAT, gave the cat to her mother who then moved a thousand miles away without ever contacting me or other family members to see if we could take care of his cat. Food was fantastic when he moved in, but unfortunately the great chef left for greener pastures and the food became low tier cafeteria food. He chose to eat in his room and meals were routinely served late and cold.
