Complete Care at Jefferson Meadows (St. Clare Care and Rehab) — Local services in Baraboo

My husband was sent there for PT he received very little because on there opinion it was to hard to help him out of bed. I would stop there after work (4pm) to check on him and most days he still have not eaten breakfast. I would ask the aids and no body know why. if the doctor stopped and i wanted to know what he have said it was a challenge no body was around that I could ask. I felt threatened by the office people because they kept telling me that if the insurance stopped paying I would have to pay out of packet, and they would not let me take my husband home. my husband finally left by ambulance with a lung infection I'm not sure if it was a complication of his cancer or the lack of care he received while he was there .my husband passed away a year ago. I hope and pray that my family never have to deal with this...

Like every other medical facility they are a little short staffed right now... But the staff that are still there are very caring and loving of the residents... Working hard to make up for the shortfall of staff... As a full time paramedic I go to multiple long term care facilities and this is one of the few that I would feel comfortable putting my parents in......

Very good nursing facility. All staff are friendly, helpful and caring. Food options are also pretty good. There is a dog and cat that roam the facility which is nice. Outdoor patio is available. My relative enjoyed their stay...
