My 91 mom was there for rehab. Would buzz, no response, she would text us at various hours how she needed to be changed and to call them. On numerous occasions, I placed a call to Elevate and asked for the nurse for my mom and would tell them she's been buzzing and being ignored. Then they would respond. Some nurses and CNAs are good but others are not - they do the minimum required. Had to speak with nursing supervisor and then nursing supervisor and nursing director. My mom had been lying in a wet diaper for some time. Which is unsanitary and unhygienic. I was told it's a new team for the past three for a month and they're trying to weed out the people who want to work versus the ones who just want to collect a paycheck and do nothing so they know there is a serious problem, But obviously nothing seems to be done to rectify it. After all, that's an Elevate problem not a patient/family problem. One day, upon arrival, I saw a CNA come out of my mom's room, I went and talked with my mother, who said the CNA had already given her a difficult time, then I proceeded to talk to the nurse and then to PT, returned to the room when my mom said that she needed to be changed again. This CNA gave the nurse (her supervisor for shift) an argument in the hallway about changing my mother saying it had been done literally 2 minutes ago. It wasn't. When I reminded her it it had been about 20 minutes, she argued with me. Another time at night, after having received several texts from my mother about being ignored, I called Elevate and said my mother needed to be changed. He told me he knew, when I asked what his name was, he he said he knew he needed to be changed and hung up on me. I told the nursing director, about the incident, she apologized, but apparently nothing was done. I have numerous texts from mom reflecting the time and what she asked for and when she finally received the assistance she required. Several times while I was there I would ask for water for my mother. It took forever to get to it. We had to help our mother with brushing her teeth, as they wouldn't even provide the water necessary for her to do so. When getting someone to assist her with washing, that was the same story. It didn't happen.The PT Department had a care plan, but neglected to tell us what specifically they were going to do or ask us what her needs were. We told them to get her to use walker as that's what she had been using for the past 20 years. Valuable time in her recovery was lost because they neglected to listen to what we were trying to tell them. One woman who had been there several times, said every time she goes there, the assistance Elevate provides gets worse and worse. Well it is rated 3 stars, that isn't accurate. My husband was there 4 years ago, while I did have to ask for items for him, the assistance and care he got...
   Read moreI would not recommend this facility for someone who needs assistance with personal care and suffering with mental issues. My father had to move to Villa because he can no longer walk. This means he needed daily assistance for personal care like washing his face and brushing his teeth. May go a couple of days without brushing his teeth.
Facility does not offer someone for the patient to talk with about health changes like not being able to walk anymore. They don't offer a professional to meet with patients to assist with their transition to a new way of life. They are not equipped to handle patients suffering from dementia or other mental health issues.
I was on the phone with him and all of a sudden the phone hung up. I called the front desk and asked her to check on my dad to make sure he was ok. She called back and said he was being moved to the nurses station. Would it have been too much to wait 1 minute for him to say what was going on instead of snatching the phone out of his hand and just hand up? They bring the patients out of the room and put them in front of the nurse's station. It's disheartening to see them just sitting there. It would be good if someone was with them interacting with them while they sit there instead of just being there looking bored. It the patient can handle themselves(walk or operate a wheelchair) they get to go to the cafeteria/break room.
Don't let the pictures fool you. They have remodeled part of the building but that area is for temporary stay patients. Long term patients get to stay in the old section of the building which has not been updated at all. They still have old furniture hanging around. A light was out in the room for almost 3 weeks before anyone came to change the bulb or see what the problem is. If you ask for assistance the staff does not respond in a timely fashion. It would be great if the long term patients stayed in the new wing.
The doctor assigned to the patient-good luck getting in touch with him. I recommend using the doctor you currently have if you can. The family tried to communicate with the doctor directly for a meeting and no results. The nurses were nice to page and send messages to him on our behalf.
There were a few good people there who treated my father with respect but it takes more than a handful of people to rate this...
   Read moreI WOULD DEFINITELY RECOMMEND ELEVATE CARE IN PALOS!!! We have a family member at Elevate Care, though it has not been a whole month yet we have already seen some progress. My husband suffered 2 strokes in May 2025 at Christ hospital during a procedure making him completely bedridden and fed through a stomach tube. Before coming to Elevate care he was at another facility (Alyia) where he didn’t progress at all and we were told not much progress would be made. Here at Elevate Care we were met by Keisha ( wonderful lady) who showed us around the facility and explained how my husband would be taking care of and showed us the huge area where his PT would take place. We were very happy to see how clean the rooms were and how things were set up for the patients coming into their facility. Everyone who has my husband as their patient has been awesome (some really went beyond for him) The Therapy team ( so awesome) work very hard with him 5 days a week, we are really happy with them . We now see my husband sitting up in a chair, talking, joking and eating solid foods( We were left to believe he would have no progress) BUT, Elevate Care showed us why they are named Elevate Care. One of his Nurses ( Sue) is awesome, she works well with him and is very patient & friendly with the family as well. Reyna ( guardian Angel) is wonderful and up beat and makes sure we are happy with his care but most of all she makes sure he’s happy and comfortable. There was another awesome nurse who checked him in when he first got there, wish I would’ve paid attention to her name because she deserves to be recognized as well. Thank you to everyone who has worked so very hard to show us good people and good care does exist. We are praying my husband will be able to continue his care there so he can get strong enough to return home. THANK YOU...
