Hand over the Medication LIST to myself&DOCTOR! "OVER MEDICATING & SEDATING" my Mother has caused "2 ER stays & now Hospitalized" & unresponsive! all is medically documented! We pray she LIVES! at first this facility was great until my mother lost a great neighbor. The staff is nice ? until they don't like you "advocating" for your loved one. The resident there is harrassing my mother,yelling at her & getting extremely aggressive. Me & my brother witnessed the harrassing aggressive behavior of her roommate on a 3 way call plus I'm a witness too as I visit. Alarming! Our family had to get inloved,we called the police for my mother's safety. I was also told before she took the room the harrassing resident pushed out 3 other residents because she wants the room for herself and is not all there. I took a chance being it's a huge room,a great view.my mother feels good there up until now she is overly medicated & now Hospitalized. the roommate started slow harrassment towards her and me. Such as feces all over the bathroom sink and not flushing her feces. My mother cleaned it up. I contacted a manager and she was told not to do it again. The lady curses at us. Following day she keeps aggressively harrassing my mother,yelling and in her face with her cane. The police told her to be civil and to stop this behavior! I want to share that my mom stays to herself. We were told to find another room! since this harrassment my mother has lost her Dignity. This is causing her and our family "severe emotional distress/depression" .In the state of New Jersey "resident to resident assaults" are on the rise and so is over medicating! The roommate walks with a cane,her weapon! The "LAW" is now involved. My mother is stage 5 kidney failure on dialysis. This caused her chest pains and tremendous fear/depression. The roommate is bringing men in the room at 4-5 am! I had no choice but to call the police. The problem should be removed not my mother. We are not tolerating this harrassment ! My mother can easily have a "stroke". The roommate knows exactly what she is doing. my mother is shy,stays to herself. The police documented all! My mother was shaking,she is there to be cared for not harrassment nor yelling cane waving scenes nor a new event now by the name of over medicating also known as elderly abuse! The roommate is a danger if she is wielding a cane! Her mouth is full of curses aimed at us as I spend time with my mother. I see a lady whom needs evaluation asap. No elderly sick person should be terribly bullied like this. Another issue I have to keep going there to feed my helpless mother. Today 6/29 no bed sheets! Severe dehydration from over medicating,mouth ulcer, lips stuck together. I had to call the ambulance šŸš‘ for real professional help and this time her original doctor placed her once again in the ER and is now Hospitalized. My mother can't respond,communicate and has withdrawn out of nowhere in just a few weeks. First were convulsion like non stop mobility issues now she is in a trance and not responsive. This is obvious the over medicating has harmed my mother!!! She is in good hands now under her original doctor in the hospital while we have asked why so much over medicating and switching of dangerous medicines and horrific side effects!!! This is Elderly Negligence at my mother's expense and LIFE! -INTENTIONAL OVER MEDICATING -CHEMICAL RESTRAINT -INTENTIONAL MISCONDUCT We are now trying to "save her Life" We are praying she survives these highly combined meds that caused this. My mother can't even talk anymore because she was being overly Sedated! This happened after I complained or when I need things done which is why my mother is there but she won't be returning to non trustful hands as of today! Intentional is physical abuse,polypharmacy is elderly abuse! Not sure but we will find out! This has caused my mother serious harm and we just pray the hospital reverses what this place has done Chemically to our mother,all her loved ones are involved for her LIFE! We are all "suffering severely" hoping the hospital...

I can’t even explain how terrible my family’s experience was at Daughters of Miriam. He was admitted from Hackensack hospital. They put him in a room with a man that screamed and chanted the same 3 hebrew words over and over. He never stopped. By the next day my father was begging to just die. It was horrible. We arranged for hosoice to come in due to his pain level since he wasn’t receiveing any care from the facility. They would only administer Tylenol. Hospice finally arrived on day 3. They wouldn’t start any pain meds until we signed that we understood that medicare would no longer pay Daughters of Miriam so it woukd be a cost of $450 a day. Once signed my father was given morphine. This was at 1:00pm. I got a print out that my father was to get morphine every 3 hours for pain. Once he was comfortable and asleep I took my mother home to eat and returned back at 5:30. On the way back I noticed I had a message from DOM asking me for payment of $450 for the day! I could not believe it. When I got to my fathers room he was crying and screaming ā€œHelp me!!! Help me!!!ā€ He never receievd his meds at 4:00 and not only that they never received his meds from the pharmacy!! No one was aware of this and no one there had checked on my father. I coukd hear him from down the hall as soon as I got off the elevator. It took another 2 hiurs to get his morphine. The whole time he screamed, ā€œHelp me!!!ā€ When I got a hold of Amy in the mist iof all this she said he didn’t get his meds because, ā€œHe didn’t askā€. No one had followed up on his care. Not DOM or hospice. They would have just left him to suffer if I hadn’t returned. Laying in a dirty diaper my poor father. But Hannie knew to call to collect Payment!!! I didn’t leave my fathers side. Every 2 1/2-3 hours he would statt to scream again. He died that night. Well really the next morning at 1:37 am. I called the nurse in I asked if I could go get my mother and bring her back to see him one last time and she said, ā€œYou better hurry up because its hot in here and I can’t keep him more than an hourā€. The next day I had 5 missed calls from Hannie looking for her $450. As I mourned my father, comforted my son and mother, my phone kept ringing from these discustung insensitive people whose only concern was $$$ and never my fathers care. My fathers screams continue to haunt me almost 2 months later. I have so mich regret for ever bringing him there. I hope he is resting easy and forgives me. Please don’t ever bring your...

I’d like to take this opportunity to review the care that my mother has been receiving at your sub-acute care facility. I’d like to say that as a nurse, I have been acutely aware of the skill and dedication of your staff as I am there every day for at least six hours. The care that my mother has been receiving is exemplary. Everyone that she and I have been in contact with has shown compassion and patience beyond what I would have ever expected. They treat her with respect and dignity. In addition to the kindness they show, their level of competence and compassion is remarkable; the two cornerstones of what I feel is basic in best practice nursing. Everyone from the unit clerks, housekeeping, nurse's aides, physical therapists, occupational therapist, social worker, speech therapist and psychiatric nurse practitioner have been extraordinary. I am so grateful to the entire staff of 1 West for making this very difficult situation a little more bearable. We are gaining knowledge and insight into her condition every day as the staff has been sharing their experiences with us. In addition to the excellent care, I’d like to recognize several people who have gone above and beyond our expectations. I’d like to thank Lori, Assistant Director of Nursing, for her coordination of care and Nurse Practitioners –Teresa and Esther for their diligent and responsive adjustments to the nursing care plan. I am appreciative to all the RNs, Brittany, Mohammed, Cleo and Lisa who’ve been helpful with explaining medications and changes in her status. I’d like to thank 3pm-11pm nurse's aide Janice for being with my mom at a difficult time of day. My most heartfelt thanks to the 7am-3pm nurse's aide,Yenesia, who helps my mom every day with her bathing and personal needs. I stand in awe of her patience. She has taught me more about my mom’s condition than I could ever learn in a textbook. And, as always, she shows my mom so much respect and maintains her dignity at every turn. I’ve been a nurse for 45 years, both bedside and as an administrator.
