On May 28, 2025, when we arrived at the facility, my father was lethargic, confused, and shaking profusely. We alerted the staff, but they were too busy to check on him. We requested that they call an ambulance. The director of nursing, Abraham, came to the room, observed my father’s condition, and stated that he didn’t believe my father needed to go to the hospital. He claimed my father was shaking because he was cold and that everything was fine since he had eaten 75% of his breakfast. We were told to wait a few hours, but we explained that we did not want to wait, and he rudely dismissed us, telling us to speak with Nurse Riley.
At this point, we were scared because my father’s condition was worsening. Nurse Riley felt we were being rude and chose to ignore us, returning to her office. Frustrated and feeling unheard, we decided to call 911 ourselves. Not a single employee came in to check on my father during this entire ordeal.
When the ambulance arrived, the paramedics assessed his condition and had to call for a different ambulance because they were not equipped with the necessary equipment to treat him. When the second ambulance arrived, they immediately checked his vitals and administered oxygen. He had a fever of 103°F. Upon arrival at the hospital, we were informed that his blood pressure was dangerously low, his heart rate dangerously high, he was septic, had a UTI was dehydrated, and his kidneys were not functioning properly. All this and not a single staff member at the nursing home believed that anything was amiss.
Additionally, my father was still soiled when the ambulance arrived, even though we had asked for him to be cleaned up during this whole situation. No one should be treated this way, and Nurse Riley and Abraham should be held accountable for their treatment of family members and their negligence in dire circumstances.
This is not the first incident at this facility. My father was also hospitalized in mid-March because they failed to administer his liver medication correctly, resulting in his ammonia level reaching dangerously high levels. This caused an altered mental status, and we found him unresponsive. Once again, no one called for an ambulance.
The staff at this facility are not responsible healthcare professionals and should not be in this field if the well-being of patients is not their top priority. No human being should be treated the way they...

My loved one was in this facility for rehabilitation. She is now on a ventilator fighting for her life due to their gross negligence.
Mrs. Riley the RN and nurse manager for the 4th floor refused to assess and help my family member in the middle of a medical crisis. She was asked multiple times to assess my family member by an LPN and she screamed at the LPN telling her that my family member was fine to leave her alone.
Later on in the evening a family member of mine asked the evening nurse manager Mrs. Gifti to call 911 as my loved one was gasping for air she said she would. 30 minutes pass by no ambulance comes instead a transport Ambulance (which is meant only for medically stable patients) showed up they took my loved ones vitals and confirmed she was in a serious medical crisis and could not transport her and requested that 911 be called for an emergency.
Mrs. Riley and Mrs. Gifti should no longer have jobs since this is how they treat their patients who they are medically responsible for. The two mentioned RNs above also forced the lpns to forge my loved ones vitals to make her look stable.
Vitals are not taken properly their machines are inaccurate. They withheld vital medication that my loved one needed and did not give it to them for 5 weeks.
Call bells are not answered in a timely matter. They over load their already scarce CNA staff with to many patients which often leave the patients sitting in soiled diapers for up to 11 hours at a time. The CNA staff is exhausted and often asked to work doubles and miss their breaks that is legally required by law to be provided because the staff is quitting due to the horrible work conditions they deal with.
The office staff is only worried about turn over rates getting patients who are not ready to leave out so they can milk the new patient’s insurance.
In the middle of a medical emergency the social worker Karen Johnson was only worried about shoving discharge papers to sign in our face while we were concerned about what was happening to our family member who was in a medical crisis.
We trusted this facility with the health and safety of our loved one and they severely dropped the ball.

Read moreAwful. Do your research before putting your loved one here. You only get one mother and one father and if you choose to put them in here for rehab after a hospital stay or long term for which there is a 3 year wait for a bed, go in and talk to some of the residents. The place is beautiful from the outside. Even the courtyard has a lovely pond with koi fish and a gazebo and they have a bbq out there and in the summer it certainly is nice. The rooms are fairly nice and newish with air mattresses and TV sets and a large bathroom all wheelchair accessable. I was very impressed with the quality and taste of the food and i live in the neighborhood and it takes a lot to impress me. Each floor has its own dining area and its well lite with natural light and picture windows.
Ok then why.. you may ask, am i giving this nursing home only 1 stars? Here is part of the problem..and this review is only a preliminary one, i will correct it one way or the other over the next week or two but for now its only 2 stars. My reasoning is this. The people that work there. They are some of the most miserable people you ever want to meet. Im yet to see someone smile at another person, say how are you today..good morning..how are you feeling..Also i noticed they are quite rough with the residents. There is reason for concern that your loved one may get roughed up some or more. Check your loved one several times daily or more while there, show up at times they dont expect you. I think they lack patience and kindness and thats sad because it has the capability of being a very good nursing home for the area. The New York state board watchdogs also gave this home 2 stars and the average in NYC is 3 stars. That is on a rating of 1-5 stars. Im sure the people who work here are underpaid which adds to their misery..and that really comes through for the residents. So then that said this is the best of the worst and thats all there is...
