Three days after my mother's surgery it was collectively decided that rehabilitation might be the next step to recovery. She was transported by Suburban transport to Onondaga Center for Rehabilitation and we drove separately. Upon arriving at the facility we found our mother slumped over in an unlocked wheelchair with no walker in front of her. She was left unattended and had to use the bathroom. Ruler #1 in wheelchair use is never leave anyone sitting in the chair without locking it. She was trying to get up when we arrived. We took her to the 2 room shared bathroom where you have to lock and unlock two doors. If someone forgets to unlock it in the night than the other person will not be able to use the facility. Second the smell of urine and the stickiness on the floor made us want to throw out our shoes. When the admitting nurse came in we, again, collectively with our mother decided she was not staying. We phoned her surgeon to get all the meds coordinated. The surgeon told us to have the facility call him and they refused. The head nurse gave us an ultimatum as no doctor was on call until the next day. They would bring her back to the hospital or we had to leave her there. We informed them that she had patient rights and since she wasn't even admitted yet we were leaving. They told us if we were refusing their care we would be reported to Adult Protective Services and they reported us. The caseworker came to her home the next day and closed the case. If anyone should be reported it's this facility. I feel horrible for those residents that have no family members to watch...
Read moreMy mother entered this facility 9/15 and was taken out AMA on 10/1 for her own safety. In a nutshell this facility is poorly managed, NO communication, LACK of care and UNCLEANLY. From day one things were not right and an email was sent to Jocelyn (Director of Rehab) who replied she shared our concerns with Rebekah, Dawn and Megan, all Directors of a department. NOT ONE of them reached out. It took over a week for someone to clean up this mess behind the side table left by a prior resident. Just think if that isn't cleaned up - how are they properly cleaning during COVID??? They may claim lack of staff but then don't take in more patients then you can actually handle safely. Call Don't Fall is their motto - taped on the room wall. Is 1/2 hour too long for a call bell to be on? My mother fell one evening in the bathroom on "something wet". She was plopped back in bed in wet clothes (probably someone's urine). The list goes on but you get the jist. The horror stories about these facilities appears to be true. It's very sad but luckily my mother is now doing fine. She's actually getting home therapy weekly which is helping more than at rehab. She's gained back the 10 pounds she lost while there because she is actually getting proper meals in her - not just flopped on her table and left. She's clean and feels comfortable because she's given supplies to wash herself daily and assisted with a shower twice weekly. Read moreI have been a Google reviewer for years now and have never posted a negative review of a Nursing home but here goes! I have been a Nurse for over 20 years and recently called by Sabina for a face to face interview for a management position and had heard this facility needed SEASONED competent Nurse Managers to try and turn this facilities SOILED reputation around and implement changes to better serve the residents needs!!! I arrived on time after a 1 hour drive. Was asked to sit and wait for 25 minutes, Administrator Charlie was on the phone. ( I could hear his conversation through a closed door) I was then approached by Sabina and told " The Administrator has left the building for the day"( even though I could still hear him talking) I was asked to go back home that she(Sabina) would call me before 4pm to setup a "ZOOM" meeting for my interview. No phone call ever received after additional VM were sent to Sabina, Charlie and the DON with no return call. If this is the way they treat QUALITY candidates for Nursing dept and residents as is evidenced by the NUMEROUS allegations of abuse, neglect etc.... in the reviews here, they may wish to consider a" House Cleaning" and reallocation of money to both attract and employ those of us who are TRUE AGENTS of change for facilities like these before they are shuttered for lack of empathetic, competent, ethical, advocates for their most PRECIOUS residents.
