UPDATE - As suggested by the owner, in their response to my original review I reached out to which I received an automated email saying that it would be addressed within the next 2 days, that took place last Wednesday. Fast forward 5 days to today - nothing, no email, nobody followed through. Seems like poor/inconsistent customer service issue is a company wide issue.

Haven't been to this location in a long time, typically things are great but today's service was unbelievably horrible. This is the first time in my life that I did not tip. Our server, Shione F (as printed on the receipt), completely dropped the ball:
She took a very long time before coming over to get our drink order, by the time she finally did we had our appetizer/dinner orders ready to go. Typically if an establishment is a little busier I definitely understand but it wasn't the case today.
No cutlery was given to us, just napkins.
Our main dishes came out first, and when they did we had to ask about our appetizer that we had specifically asked to have first.
I waited about 5-10 minutes after asking to actually receive any cutlery, same thing with ketchup and their usual assortment of add ons that are typically dropped off at the table prior to the meal with the cutlery.
Our server apologized about the appetizer but never offered to take it off our bill. When we were more than halfway done our meal our appetizer finally arrived on the colder side and neither of us had any appetite to finish that.
The periodic check ins were far and few and she definitely gets points for owning up to the mistake but she did absolutely nothing to rectify it.
Upon finishing our meals, another server came to grab our plates and we then waited around 15 minutes to ask for our bill. Shione was nowhere to be found and we ended up just going to the front where we waited for another 10 minutes for the host to arrive and then we still had to wait for Shione to come and print our bills.
The meal was mediocre at best but I think that had more to do with our displeasure with the service and then having to wait so long to receive any food...

I picked up the menus and they were all dirty with black specs and grease spots, waited quite a while and noone came by so I brought the menus to the front, waited again and left them there and took new menus. We ordered nachos, pizza flight, spaghetti and meatballs, quesadilla, and drinks and after taking out order we asked if there was happy hour and the waitress said you didn't get a menu? We said no and then she brought the menu. On the hh menu, I saw the nachos on the menu so I thought great we got a discount on them. We also ordered the deep fried pickles after seeing the hh menu. Our food came and then it was quite a while and we still didn't get the deep friend pickles so I asked about it and the waitress said that's cause they still cooking it....yet she came back right away with them...I think she forgot. We waited a long a time for anyone to come to us as we were done eating and wanted boxes for our take out. Then we asked 2 different people for the bill...noone showed up the we finally saw our waitress and we asked for the bill. We checked the bill to see if the nachos were hh price and they were not...when inquired she said it was the smaller size. It was a very frustrating and disappointed visit. The waitress apologized about the hh menu but at this point we were in the middle of paying and we were just so over the service. We told her menus were filthy and we had to get our own menus, we had to ask for the bill 3 times, the hh menu was never provided to us and just waiting for anyone to help us was poor. We have been here before and the service was better.Won't be returning...
