I went for mamogram at covina office. When I checked in I was asked for my iD and insurance card. I handed it to her and sat down with clipboard to fill out paperwork. I handed back clipboard and she took clipboard and asked me to have a seat. At this point i thought I should ask for my Id and card back but I figured maybe they were still processing. My paperwork. When I asked after my procedure they proceeded to tell me they already gave it to me. I looked through my purse, my wallet, myself, showing them i did not recieve anything back. All they could offer was a callback if found. Now I have to worry about Identity theft during the holiday and go to DmV for new ID. Update: I'm not sure if you are the owner of this facility because apparently nobody at the Covina office knows what you are talking about. I never used the bathroom and even the office staff confirmed that I would have needed a key to get in bathroom. So I didn't inadvertently leave anything in the bathroom. I received call from front office staff stating they found my ID and insuranace cards in between files. So either someone lied to you, or they lied to me or you're lying to me. What can I really, review?
I can not express how grateful, I am with all the ⚕️ healthcare staff meaning all the employees from the entering the center reception desk, check point, imaging procedure to exiting the center premises.
Albert, thank you for assisting me with medical records, procedure reports, imaging procedure CD's, scheduling appointments, and so forth. Albert has excellent client relations skills.
Now, the business of radiology imaging is a great healthcare business, at the end of the it is a healthcare business. Excel's to thriving to meet the clients health needs.

Edit: Weeks later I'm still having trouble making an appointment! I tried the online scheduling tool and the system wouldn't upload the pdf scans of the papers of my doctors orders. So I had to try again over the phone. Called the 909 number FIVE TIMES and couldn't get in touch with anyone at all any of those times (it just kept automatically hanging up after ringing a while). Finally called the 626 number and while I was FINALLY able to talk to someone, they ended up just telling me to email the authorization papers and call AGAIN Monday to set up the appointment.
