Convenient 2 lane bridge with a walkway on the side that you can cross to get to the Bronx or Manhattan. Entering Manhattan via vehicle, bearing left at the fork at the stop light getting off the bridge you will be on 135th and Madison. Keeping straight you will be on 138th and 5th. By walkway to get to 135th enter on the left (traffic going opposite of you to the Bronx) side. To get to 138th (traffic going in the same direction as you) enter on the right side.
To get to the Bronx via vehicle the entrance is on 135th and Madison. By walkway the entrances are on 135th and Madison and on 138th and 5th. When you get off the bridge you will be on E138th street.
By vehicle, the bridge can be chaotic at times especially during rush hours but is probably the best bridge for traveling between the boroughs.
By walkway, not nearly as much traffic but walkways are narrow until you reach the overpass.
