Osseointegration (OI) offers the possibility of maintaining a functional shoulder joint and requires patients to follow a strict rehabilitation protocol to ensure a successful outcome. Specifically, graded weight training and axial loading are needed after OI prior to beginning prosthetic use. Weight is increased (50g-150g) weekly on the training prosthesis until the weight of the final prosthesis is reached (usually, 800g). Additionally, an axial load is placed through the abutment site by loading an analog scale, increasing 2-4 lbs a week. Tom’s first training session with his myoelectric upper extremity prosthesis! 🦾Tom underwent treatment with the OPRA™ Implant System and TMR (Targeted Muscle Reinnervation), a surgical procedure which improves muscular control of the upper limb prosthesis, in 2023. He has been training with the OPRA Transhumeral Training Prosthesis and has met the requirements to begin training with the full length prosthesis. He was fitted with an osseointegration-style COAPT cuff embedded with an array of surface electrodes contacting the skin to that allow for muscle pattern information to be captured. Pattern recognition mapping was programmed into Steeper Elbow and i-Limb Quantum hand.🧠 📝 Our take aways from day 1: when practicing pattern recognition control, there is no need for extra-hard muscle contractions. The contraction levels should be light to moderate. Trunk and pelvic rotation play an important role in balance, stability, and forward momentum during gait. ⛳️ The use of external cues provide a clear reference point and guide attention to relevant aspects of the movement 🔬They facilitate the development of proprioceptive awareness and help learners become more proficient and consistent in their movements. Here, we are focusing on left transverse pelvic rotation / forward rotation and weight shifting over the prosthetic foot. 🚪Initially, the task is performed without resistance until the patient is consistent with his performance. ⏩️We then progressed to using resistance band for strength and power production, along with tactile cueing. 🦿The patient then walked with PT assisting with trunk rotation / reciprocal gait patterning. The patient came to PT using a walker and left using forearm crutches.
