honestly, if i could give less than ONE star, i would! its a horrible place to send your loved ones! first few days everything was clean& everything was on point but soon after being admitted, everything took a turn for the worse! my loved one was not being bathed, teeth NOT being brushed...bad breath& grunge on teeth, her room was not being cleaned, dehydration, her things were coming up missing& their only response was "i don't know what to tell you" after numerous attempts to recover her personal belongings. i held her hands and the smell was terrible, turns out she had feces under her nails(i assume from touching herself during a diaper change), she always keeps an infection...including UTI infection& MRSA at peg site. they don't bathe her until the day of or the day before there is a doctors appointment. she has fallen out of the bed...floors are filthy, bathrooms are filthy& when you notify the staff, both staff& the housekeepers get an attitude about what they're supposed to do. most of the staff are always sitting around doing nothing& again, get offended when you ask for cleaning or updates on your loved ones. i know that there will most likely be a problem or concern with every nursing home or rehabilitation center that you send your loved one to but this one is by far the WORST! please consider all other options BEFORE you choose this place! #UPDATED WITH PICTURES OF MY LOVED ONES ROOM, AS WELL AS HER GOWN THAT THEY SAID WAS CLEAN! THERE IS ALSO A BLISTER THAT WAS ON HER FOOT THAT NO ONE NOTICED& IT WENT UNTREATED FOR SOME TIME UNTIL IT POPPED ON ITS OWN...THIS WOUND WAS RECENTLY DOCUMENTED, AS WELL AS RASHES& "OPEN WOUNDS" ON HER BODY (UNDER BREAST, UNDER ARMS, BETWEEN THIGHS& RECTUM AREA) THAT HAD TO BE TREATED BY DOCTORS AT OUR LOCAL HOSPITAL...PROUD TO SAY SHE IS NO LONGER AT...

I was able to observe my mother during physical therapy. This was a great experience. The PT/OT room is roomy and bright with all the windows. It has a nice vibe. The PTA was very friendly and welcoming and invited me to attend the session. My mother has been here 2 years and this was the first time I saw her do anything physical or stimulating. It comforts me to know that she is getting to partake in this even if it is only a few months at a time.
When I first arrive to visit my mother, she is usually sitting in the dining room asleep with her head leaned over in an awkward position. I have asked that she not be allowed to sleep sitting up several times. During my visits (every 2-3 months) when I observe the dining room, the residents are all required to sit with nothing to do. No stimulation. No TV. No one interacting with them. Just sitting for hours. I wish the staff could take turns interacting with them - throw a beach ball, swing a hoola hoop, play an instrument, put on music and dance for them, just anything to stimulate their brains and make dying more enjoyable.
I have noticed a CONSIDERABLE positive change in the cleanliness and atmosphere in the last 2-3 visits (maybe 6 months). My mother’s room is clean and neat. The entire facility smells better. The new(ish) social worker, Heather is AMAZING! Please keep her! She is on the ball!

Read moreMy mother has been a resident at Golden Creek since June 27. Upon her arrival, it was agreed that her CNA would assist her with restroom visits every two hours. However, this practice ceased once she was moved to a different room. After a few months, I requested that they document her shower and bathroom times, to which they agreed. Unfortunately, they only followed through with the shower documentation.
Yesterday, my mother called me, upset because she had soiled her diaper. It was then I discovered that they were not keeping track of her restroom visits. Later that night, a nurse informed me that my mother had developed a rash. Today, I visited the facility and provided them with a spreadsheet to document bathroom times. I emphasized that I don't mind how they document it, as long as it is done. My priority is ensuring my mother receives proper care.
Regrettably, I have come to the conclusion that I need to find another facility, as Golden Creek refuses to document this crucial aspect of her care. It seems they are more concerned about potential repercussions for neglecting my mother's needs than maintaining accurate records.
