WARNING..I only gave one star because you can't give 0. This rehabilitation facility is a death trap for your loved one. My mother went there released from hospital on a Tuesday afternoon & almost dead less than 48 hrs later. I was told by Liz in admissions all rooms have window for visits during this difficult period of not being allowed in the building. I also checked on NYS website where all Nursing home/Rehabilitation facilities are mandated to ACCURATELY list COVID 19 cases & deaths . Berkshire listed 2 ...which is false. This was confirmed by a employee who was leaving. She informed me the number was much higher..closer to 40 BIG DIFFERENCE This information and the ablility to have a window visit was my deciding factor in choosing this location for my mother. What a mistake that almost cost her life due to lack of concern, truth, & NEGLECT by staff and administrators too. My family, myself included arranged a so called window visit thru a narrow glass door and were horrified by what we saw. My mother was sitting slumped over in a wheelchair in a sleep coma. I was told she was awake & eating a hour before..LIES TO SAVE THE LACK OF ATTENTION & POOR CARE. We called Dr. Haque & called inside spoke to Colleen ( nurse) several times and asked how did this happen & what are you going to do about it...they had no plan. They would have waited another day I was later told by Kathy nurse administrator & Teresa nurse . I insisted they call a ambulance & return her to Good Samaritan hospital. Colleen thought it was a good idea to call a transport instead of Resuce ...go figure she needs retraining for sure. My family waited outside for Rescue & filled them in on the situation before they would by fed lies by all employees to cover up the neglect. Rescue took her out on stretcher in a lethargic comatose state. They told me all her vitals were low & I made the right decision. My daughter noticed that she had something hanging out her mouth as the closed up ambulance. After she received her emergency and I received a call ,I was told that she had a mouthful of food that wasn't swallowed and she was left to sit like that for an hour. If I had not got there she probably would have choked to death on it. They were force feeding her & did not even have her dentures in her mouth. I had brought a bag of her clothing Mr immediately Tuesday after she arrived with an inventory list and Thursday when they brought her to the window half-dead she was sitting wearing somebody else's clothing absolutely unacceptable. I can't even think about if we didn't show up that she would have passed away wearing somebody else's clothes Disgusting Behavior. I went into the lobby and insisted that I'm not leaving until I have her own clothes return to me in any item she brought in because I did not want to step foot back in there ever and I suggest nobody else should either. We were greeted outside by Kathy Teresa and Colleen who stood there trying to convince us otherwise and they were not doing anything wrong and made a bunch of excuses and lies and just insulted our intelligence and at that point I wasn't having it. I also received two phone calls the next day trying to make nice and light of the situation and not once they asked me how is she now doing. Over the past 20 years my mother has been in more rehab than I care to mention and this was absolutely the worst. The only person there who shows any human qualities is Jennifer the social worker she went out of her way to try to help me and correct the situation and is empathetic person. My mother has been sitting in Good Sam Hospital in that comatose state for the past two days and they have told me it was from lack of nutrition severely dehydrated and no medication administered how long were they going to let that go on before somebody stepped in to take life-saving measures. I just thank God we showed up when we did and do not bring anybody you care or love to this facility just a warning! Thank goodness doctor call today she's starting to get more alert. Read moreALERT! NOT A GOOD PLACE My sister was sent here 9/15 for OT and PT. She also had severe shoulder, neck and head pain. She was sent directly from the hospital so I’m sure she came with dr’s orders for rehab. She was told the therapist would have to evaluate her but it was the weekend so it would be Monday. Meaning they left her in bed because they weren’t able to move her until she saw the therapist. She asked the aide for a toothbrush and was told she will brush after she eats not before. That’s how they did things there. I brought her 2 bags of clothing on 9/19, after the debacle of labeling one bag was delivered to her neighbor’s side while the concierge ran around trying to find it. (minor at this point ). Each time the nurse’s station was called, my sister begged us not to complain she said they were not nice to her but would not give specifics. By Thursday she sounded better and was still able to answer the phone. Then on Friday she’s not able to grip anything she said her hands stopped working. Her grandson visited and went to the nurse’s station to find out what was going on. He felt she was getting worse. On Saturday I arrived around noon to find her still in bed because no one came to help her. She was totally incapacitated. I went to the nurse’s station and spoke to Teresa who informed that she had refused her pain medicine and she couldn’t get her out bed without my sister’s help. I asked what time was she offered pain medicine before Teresa could answer Kimberly interjected she had administered pain medicine at 6:00AM Then Teresa said 4 hours later. Again I asked what time, Teresa gave the same response 4 hours later. I said well it’s after 12 when was she offered pain medicine again? I was told to ask her if she wanted pain medicine I came back and said yes, because she wants to get up. Teresa brought the medicine, left returned shortly with Kimberly and they began to get her up. ( obvious retraining was needed). Then in walks Mark who joined them trying to get her in the wheelchair. Eventually they did after pulling and pushing but she wasn’t comfortable her head was down and she told them. But they left like they had done something great. Later Mark returned and she said again I need my head back he got someone to come in to help adjust HER using a pillow under the arm she them asked him to adjust the back of the wheelchair which could recline to different positions. ( why they didn’t know that). She finally found a comfortable position for her head and neck. Teresa came in and asked if I wanted to feed since she no longer had the use of her hand. I did. While there Dr, Smith came to inform her that she had an infection possibly a uti and he would order antibiotics and a saline drip and Ensure because she wasn’t eating well. I left arrived home to a call from Mark asking my consent for her to receive the antibiotic and saline. I told him I was there when she gave the doctor her consent , so he changed it to he was just letting me know what was going on. Sunday 9/24 around 10:45 AM my son came and got me out of church service because Mark needed my consent to have her transported to the hospital and I wasn’t answering my phone. I called him back and asked was she incoherent and did anyone ask her. I told him I was on my way arrive walked passed Mark in the hallway outside he did not say a word nor did I. The paramedic was there taking vitals I identified myself. The EMTs came in took over and transported her to the hospital. My son and I returned later retrieved her clothing and no one has contacted me yet. It’s Monday, 9/25 @10:22 that I’m writing this. Do Not put your love one through this torture. My family member was sent here after an ICU stay, and he left on his last day after gasping for air, begging for help, and being left completely alone by the staff here. Despite voiced family concerns and visible signs of distress, the nurses refused to call an ambulance for hours. My family found him on the verge of death, and it took them screaming and demanding help before the staff finally acted—far too late. He was rushed to the ICU, where after fighting his best, he eventually passed away.
The way he was treated by this facility was not just negligent—it was cruel, inhumane and utterly indifferent to human suffering. The staff here don’t provide care—they play politics, dodge responsibility, and actively ignore the needs of their patients until it’s too late.
Patients are left to suffer in silence while nurses lie to families about their condition, pretend to provide care, and refuse to take even the most basic, humane actions—like calling an ambulance for someone who is visibly gasping for air and begging for help. That’s not an exaggeration—that’s what my family witnessed with their own eyes.
There is a culture of silence and fear here, where some staff quietly acknowledge wrongdoing but refuse to act due to who knows, while leadership hides behind red tape, fake investigations, and total avoidance. They ignore family concerns, dodge accountability, and treat dying patients like problems to be managed—not people to be cared for. This place does not care about human life. You would treat a stranger on the street better than they treat their patients. They have no compassion. It’s hard to imagine what goes on in the heads of the people who work here, as watching someone suffering dying and refusing to act is an unimaginable concept, but from what I’ve seen, they care only about money, image, and power games among staff and management.
If you are considering placing a loved one here, please, for their comfort and truly for their survival, look elsewhere. We trusted this facility with someone we loved deeply, and they failed him in the most unforgivable way. We will never get those hours back. We will never forget the suffering we saw.
