I have no tattoos and am old enough not to care about getting naked in front of strangers, so from my perspective a solid experience.
Don't be tricked by the vending machine on the 3rd floor, it does not have milk! You have to go down to the first floor and there's a fridge close to the checkout counter with milk (I think).
I jumped into the cold bath and the hot bath a couple times. If that (along with being in a closed room with a bunch of people) causes me to get symptomatic covid-19 later on my vacation I may rescind my review, though no one was coughing.
Also, kind of crowded at around 1800 on a Tuesday.
There is a towel rental fee in the likely event you did not bring your own.

Went for the public bath and 500 yen in the middle of Tokyo is not bad. Went at 7pm and it was crowded, but did not have to wait for spots to open up. Has three different baths with different temps (basically hot, very warm, and very cold).
The “lounge” offers food and drinks which were costly, so we did not buy these.
Overall, I think 500 yen for just the public bath...

You put your shoes in the locker first and then talk to the people at the front desk. You can rent your towel here and then go to the locker room to change. In the women’s side, there are lockers to put your stuff in and a room with moisturizing lotion and a hairdryer. You have access to three pools (small hot pool, big medium temp pool, and small cold pool). There is also a sauna for an extra charge.
On another floor, there’s a nice restaurant/lounge area to relax in.
