Don't go unless you have a "Thanatos wish" . From Administration on down the service and level of care is negligent, harrowing, horrific, heinous and outright substandard.
Kaiser" rail roaded" my family and I to place our mother there touting it as the best on their list of Skilled nursing facilities.
The level of care was horrific my mother became tachypnic after they administered an antibiotic then my Mom had a change in level of consciousness. I pressed the call light and for 15 minutes no one came to the room I then yelled out onto the hallway and the CNA's and RN nurses just looked over their shoulders and continued what they were doing. I screamed "HELP please someone HELP my mother needs help!!!" I placed a cell phone call to my sister who is an RN nurse and I also called a friend who is an EMT for assistance. My sister guided me through a series of assessments for immediate discernment of seriousness of condition. My sister asked that I not call 911 unless someone doesn't show up within next 3-4 minutes.
My EMT friend who lived close by came over within 5 minutes of my call to him. He then checked my mother and thank GOd she was doing better by then. After 30 minutes the LVN nurse showed up. She came in with a medical chart in hand placed it on the empty bed next to my mom's bed and stated that the reduced level on consciousness was due to mom's infection. The nurse left and after an hour as my mother's condition got better my EMT friend noticed that a medical file was on the empty bed next to my mother. We went to investigate the file we noticed it was my mother's medical chart It was left out in the open and a clear VIOLATION of HIPPA law.
My mother had the diagnosis of dysphagia (difficulty swallowing) the medications nurse for every shift would ask me how she should administer my mom's medications. I would educate them on my understanding that they should be crushed or contents of capsule be poured out if possible but that the Dr. should be asked.
On one occasion the medicine nurse asked about medications administration and her breath smelled heavily of alcohol. I advised her that due to dysphagia she should ask doctor however, crushing is advised and pouring content out of a capsule was the modality of administration. She still gave a huge capsule to my mother to swallow. After the nurse left the CNA in the room helped my helped to clean when we noticed that a huge capusule was on the towel that was draped on my mother 's neck .
Pills have been found under my mother's bed. The CNA's will pull on resident's diaper or articles of clothing to transferrer them from bed to chair and visa versa.

The CNA team on both day and night shifts has been consistently caring and compassionate, supporting not only my mother-in-law but also my wife and me during a challenging time.
The business office team has been outstanding in helping us navigate insurance coverage and financial responsibilities with clarity and transparency—something that eased a lot of stress.
The dietary staff deserves praise for their attentiveness and willingness to cater to specific dietary needs, especially in preparation for a possible upcoming surgery. They took the time to explain her meal program and answered all our questions thoroughly.
Housekeeping has kept her room clean and tidy, and we appreciate their efforts in making it feel like she’s in her own home.
Lastly, I want to recognize the incredible work of the rehab department, which has played a crucial role in my mother-in-law’s recovery. Michael, Dee-Dee, Daniel, and the entire rehab team have not only delivered expert care but have done so with encouragement, motivation, and empathy. They provided clear communication about her treatment plan and recovery goals, empowering her to stay focused and not give up.
We fully expect that my mother-in-law will return to Canyon Oaks after her surgery, and it gives us great peace of mind to know she will be back in such capable and caring hands.

Read moreMy mother was brought here from Kaiser Permanente. After a rather lengthy surgical procedure, she was in Canyon Oaks for 21 days. At day 3, they were already discussing discharge when even simple communications with her were anemic at best. We were being lied too by the physical therapists, the Kaiser doctors, and nurses stating that my mother had been improving physically. They were stating that she was able to walk 40 feet assisted. Ironically at the 18th day of her stay we were called by the Kaiser doctor who relayed to us that she had plateaud and that she was strong enough to come home and would be discharged on the 21st day (insurance would no longer cover the stay after 20 days). On June 23rd, 2020 at 12:00PM I was scheduled to pick my mother up who was forcibly discharged from Canyon Oaks. Prior to discharge, I was met by a nurse that produced paperwork stating that she was able to walk 50' assisted. I gathered all supporting documentation from Canyon Oaks prior to signing anything. So they finished with her lunch and bathing, and wheeled her to the lobby at 1:00PM. I requested assistance getting her into my vehicle which was parked right in front of Canyon Oaks lobby. My mother couldn't stand up from the wheel chair. She couldn't even pivot once assisted by two nurses after they got her up from the wheelchair. It took three of them to get her in my vehicle safely. Once I got her home, it took everything I had in me to get her out of my vehicle and into the wheelchair with caregiver assistance. She is not able to walk, even with assistance. Canyon Oaks, their therapists, and Kaiser Permanente doctors lied over and over too us regarding improvement just to get that insurance money for 20 days, and booted her from the facility with no improvement whatsoever. They don't care about their patients. They will lie to the family to get them through the system. July 24th. 2020 (24 hours after release) my mother has taken a fall in her home. 911 was called and paramedics had to get her stabilized and into bed.
