My aunt fell, broke her hip and needed rehab after hip surgery mid January. Her insurance limited her choices, but after careful review and actually visiting in person Alameda Healthcare, the decision was made to chose Alameda Healthcare. She's 85 with dementia, she can communicate but couldn't remember she just had surgery. The staff knew she was a fall risk and placed her in a room where we were assured a staff member literally sits outside her door at night in the hallway to ensure she doesn't wander and fall. 48 hours later she fell at night, broke her arm. The outside imagining company who took her X-ray at Alameda Healthcare didn't correctly read the X-ray, they said "no fracture", physical therapy continued despite the swelling and bruising in the arm. Ten days later because my aunt was really complaining of pain, she was taken to ER at Alameda Hospital, ER discovered the break. She was moved to a room closer to the nurse's station so the nurses could watch her more closely. She fell again, hitting her head on the door. This room had a broken toilet that took an entire weekend to fix. The staff put adult diapers on my aunt ( she is totally able to use the toilet & never wears even a pad nor pull-up) in lieu of a working toilet. After complaints the staff put a commode next to her bed. The morning after the toilet was fixed, her hospital bed mechanism broke. We were assured she was drinking a drink similar to Ensure 2 times a day (because she wasn't eating), but she was never even offered Ensure. Even after the toilet was fixed, the staff put diapers on her again, family arrived mid afternoon on a Saturday to visit, she was at the edge of her bed trying to get up to use the toilet. Family asked staff for help and pushed the call button, no response (although several staff members right outside her room were asked for help - no one agreed to assist), her daughter assisted her to the commode, diaper completely soaked. After questioning the staff it was discovered she had no assigned nurse (by accident) for the day. Shortly after this incident, she got pneumonia, a few days after the pneumonia diagnosis we learned she was exposed to listeria. This had to be the longest and most frustrating 6 weeks of rehab I've ever encountered, how could she get more sickly and frail as each week went by due to neglect. I waited weeks after her discharge to write this review, hoping once I stepped away from the situation I'd be able to write a clear, logical review. She's now in memory care in an assisted living in Hayward. Thankfully doing much better. I realize my aunt's dementia makes a rehab at any facility a huge challenge. But what was most exasperating at Alameda Healthcare was the lack of ownership. No one seemed to understand what taking responsibility means. One particular RN was at the nursing station closest to my aunt's room almost daily, but she was there on her break, her reply was "you need to find your charge nurse" any time we posed a question. The case worker, dietician, lead administrator were almost impossible to track down even when we were in the building (forget trying to reach them on the phone). Even when an appointment was made to meet for a one on one, seldom was the appointment successful. The most dutiful nurse I encountered was a contracted nurse who usually worked at Oakland Kaiser - I needed some paperwork, he walked me to the staff member who could help me and said "I won't leave you until you get what you need." This facility does have employees who care and have abilities, I saw some wonderful therapists and LVNs, RNs, one RN in particular was so patient and kind to a long term dementia resident. My aunt had one therapist in particular who really looked out for her, Anette. The attentive and competent care was random, not consistent enough to enable my aunt to make progress. Mrs.Casandra Directer of Nursing is extremely Personal Professional polite and emence amount of patience. And wow amazing while her office being renovated while she was still resuming to work! That for certain should be rewarded though obviously the rewards going to be brand new office. Maybe they could give her a window in the office! Especially because drills saws hammering and was still on top of overseeing of Patients care properly taken care of no matter her working circumstances including her private office space and with all that doing interviews!! She deserves a raise!! Ms.Aisha whom works under believe Liza or with LiZa deserves promotion and raise! Believe possibly her position has to do with front of house and administrative! Upon arrival she was kind enough and went out her way to give tour, where and what each department is, greeted me while setting up room for patient with belongings and I didn't even ask! She took charge in introduction of facility and operations on all behalf of directors well! Professionalism is her experty. I have Evan observed her from afar take initiative to calm not only staff down when stressed but patients and Evan visitors down! And reassuring with constant positive proactive in nature! Though wearing a mask 😷 you could see and feel her smile and is genuine and takes a lot of pride in herself and job Mrs. Aisha needs to be recognized a clear explain of what front of house administration should be! Ms. Aisha needs be promoted and raise. Overall everything is pretty good regarding care of patients however #1 complaint is all rooms need to be hospital clean sanitation code at least twice a day! And all CNA or LVN need to be trained and is unexcusable to not be giving patients showers at least three times a week and Sponge bath everyday! Read moreI am at a loss for words to describe the frustration and helplessness I’m feeling in this situation. My brother has been in a coma for two years and had been receiving horrible care at the Alameda Health and Wellness Center. You would think that such a reputable location would treat its patients with the utmost respect, but unfortunately, that has not been the case for my brother and our family. After noticing signs that my brother may have been physically mistreated, we decided to install a camera. What we witnessed in the footage was absolutely horrifying. The charge nurse, who oversees many others in the unit, is responsible for this abuse. The anger I feel is indescribable, but I know that it’s my right—and my responsibility—to advocate not just for my brother, but for all patients in similar conditions. This nurse should never be allowed to work as a registered nurse again, and she must face serious consequences for her actions. No one should ever endure this kind of mistreatment, and I am determined to make sure she is held accountable.

The 4.2-star rating for this facility is misleading. Out of the two places my brother was transferred to, this one was by far the worst. Management seems to cover up the issues.
I strongly encourage others to remove their loved ones from here, just as we did. Since leaving, my brother has been doing amazingly well. He was taken off the ventilator, despite one of the respiratory supervisors saying he never would. It's frustrating that someone would make such a definitive statement. Only God truly knows...
