0 stars. DO NOT SEND YOUR LOVED ONES HERE. THIS PLACE IS HORRIBLE. My mom spent 77 days for rehab. During this time very little progress was made and they actually made her worse. When she had a bowel movement they would not clean her for 5-6 hours at times. Multiple phone calls to get someone to help her only to be told by the receptionist "there's no one here what do you want me to do about it". Issues every day.
She got sepsis and had to go to the hospital. Came back same issues. Again not being cleaned for 5-6 hours 2 days after she got back from the hospital.
Being waken up multiple times a night. Extremely hard to schedule visitation appointments. Visitations were 20 min and you were watched for some reason. It took days of calling/emailing Olubukonla the recreation director to schedule visitation appointments.
Later my mom ended up in the hospital again after being told multiple times by family to do bloodwork on her because we were suspecting an issue. We were right. She was anemic, and according to the hospital one of the issues was she was getting inadequate nutrition.
Philip who takes care of the ventilators and trachea is a lot of talk when family is around but no action and I suspect limited knowledge of his field. The outside consultant that works with Phillip was very good and caring but is limited by Phillip in my opinion. The social worker Debbie was always respectful and tried to do what she could but there was little she could do. The nursing director Dorene would never return phone calls or emails and rarely addressed the issues. She was rude and passive aggressive on occasion. The facility administrator Derek did not respond to our multiple emails and calls in any way.
This place in my opinion does more harm than good.

   Read moreDo not send your loved ones here! The nurses don't know what they're doing. My dad was sent here after leaving the hospital because he had a PEG tube. He is now healthy other than having the PEG tube and needing to be cleared by the speech therapist to consume food. He was at this place for less than 24 hours and they messed up everything! The beds were uncomfortable, it was so indented that he couldn't even roll himself around. They had to inflate the bed during the day so they had him sitting for 2-3hrs on a chair which he didn't have the strength to do. They ended up putting him in a chair/bed for the 2-3hrs so he was more comfortable. When they needed to start the PEG feeding, his nurse was uneducated. The PEG has 3 ports and the nurse could not fit the syringe into the middle port so they forced it, which caused the PEG tube to slip out of place. Originally the outer portion was touching his stomach, but they ended up pulling the tube out by an inch which is a lot! Another nurse who stated they had 17 years of experience came to help and whipped out a SCOSSIOR attempting to cut the line! In which we refused and requested for him to be sent to the hospital. Somewhere among the chaos, the nurse attempted to flush the PEG tube to make sure it was ok, but we heard an audible "pop". After arriving at the hospital, it was determined that the PEG tube was misplaced and the nurse popped the balloon that was holding the tube in place. My dad was in so much pain after even with the smallest touch and they still attempted to "fix" the situation by cutting it or whatever. I KNOW that we learn at least the bare minimum about PEG tubes in nursing school. Or at least the nursing home should teach their nurses the proper way on how to care for patients with these medical equipment. I would not trust any of my loved ones in this...

My father was transfer on August 17, by September 7 some one call me to informed me that my father had fallen from a chair, where he was getting transfer. on September 8 my father die. The department that takes care of the appointment is the worst, the never called me back on time to make appointments to see my father, the lady was always on vacation. They never call the ambulance to take my father to the ER the day he fell, they told me he was ok, next day he was dead. No communication, they don't follow up with you, they don't call you back even when you leave several messages. Only I'm sorry your father is not responding, he doesn't have any...
