Port of Subs — Restaurant in Nevada

The pastrami on my sandwich was the worst deli meat I've ever been served. All of the veggies are wilted, see photo. While I was waiting for my order one of the employees pulled out a container of shredded lettuce and covered over the wilted and browning lettuce in the bottom, which I'm pretty sure is a health code violation. I had to repeat myself a number of times when they listened to me at all, they would just start making a sandwich without asking how I wanted it; ended up with the wrong cheese on one sandwich but after awhile it's just not worth the effort. One employee asked if my grilled cheese was cooked enough I said no so she put it back in the oven hit some buttons but never turned it back on and didn't ask a second time. Neither employee knew when they should replace their plastic gloves wearing them like a part of their uniform, another health code violation and why I will always think of this chain as the Port of food...
