I was in the new urgent care office on 8/16/2025. It was a super quick in and out. One big problem happened though, which I want to share so others can be aware.
Today (8/19/25), I received a message in MyChart that I had a new test result. I opened it as I didn’t have any tests done other than an X-ray. I looked and it was a test result for someone else with my name tag and insurance info on it.
I immediately called and spoke to someone at Duly who then transferred me to the Hoffman Estates office, as that’s where the persons who’s tests results I received is out of. The guy on the phone clearly didn’t understand what I was saying because the nurse who I spoke to thought I was calling on behalf of the guy, whose test results I received.
I explained the situation to her and she was VERY confused how the Urgent Care staff did this. The test has a timestamp which shows a time BEFORE my appointment even started.
She apologized for the confusion. My biggest concern is my insurance is being billed for a test that I did not have done.
UPDATE: I spoke with the Duly Compliance department. They removed the test result from my chart and then told me to delete any and all records of this. I was informed my insurance should not have any record of this. So all is...

When I called to see if we could still meet with the MD (we were close by) I was told I would have to wait 6 months to see the doctor and pay a $50 cancellation fee. There was no concern or care - that’s just how it is. I felt like I was talking to a robot. I get the feeling since Duly took over that there is no concern for the patient any longer it’s all about $$. I realize the doctor must be very good which may cause the wait - but the way the call was handled was just insane. I have 2 major medical issues and really can’t wait 6 months. My wait time in the office was minimal. Once I was in the examining room, after a few questions with the nurse, I was off to get x-rays.
