If I was able to give this facility 0 stars I would, they’re lucky they’re even getting one. Let me start off by saying the facility looks nothing like it does on the Internet. My mother was there for about two weeks & I hate that she even had to deal with the horrible care that she did. The first week she was there it took them about two days to get the proper pain meds together for her. Day 2 the “nurse” That was handing out meds had taken the fentanyl patch off of my mothers arm when she wasn’t supposed to. I told her it was a 72 hour patch and she said “no it only stays on for 12 hours“ which is absolutely incorrect. I immediately went to the nurses station, told the head nurse that she took the patch off. Sure enough within five minutes a new patch was put on. By day 4 I was having a heated argument with the nurse supervisor because she thought it was appropriate for my mother to sit in a soiled diaper for over 4 hours because handing out food was more important then changing a patient. They do not have enough staff to take care of these patients properly & I feel horrible for the ones that do not have family to check in on them everyday! By the second week my mother started to become very swollen and was having a hard time breathing. They sent in Dr. Ike & he advised me that he would give her a steroid shot to relax the lungs. I asked that they do a Doppler to make sure there were no blood clots as well as another chest X-ray. That took 2-3 days because they had to “order it”. Testing came back with no blood clots but they were going to treat her for pneumonia. I came back the next day to the most botched IV placement in her upper arm. The moment I saw it I knew it was not supposed to look the way it did! I asked the head nurse and she said “oh it looks like that because she is swollen but we will change the dressing”. Never happened! Thursday the botched iv was still in her arm looking crazy! Asked for the doctor to come in to look at that & her foot because it was a shade of purple. When I said “her foot is purple” he said “no it is not, that is not purple” so I said what color do you think that is? The young man who was shadowing him on his rounds said “a shade of magenta” & the doctor said “no it is not!” When it clearly was. By the time Friday came I was texting her to see what she wanted for breakfast because I would bring her food due to the disgusting food that they served & she was not answering. When I got there she was struggling so bad! Her movement, her breathing, everything! I said to the medication nurse “I think I want to send my mom back to the hospital” & she said “if this was my mother I would 100% do that” & that’s what I did! They sent in Dr. Ike and he spoke to me outside of the room trying to convince me keep my mother there until Monday while she was clearly suffering and in distress! I said absolutely not & told him to set up the transportation back to Mercy. When the paramedics arrived I immediately showed them the IV in her arm & they couldn’t believe their eyes. I told them it’s been like that since Wednesday & they were flabbergasted! The look on their faces said it all. Upon arrival @ the ER they told me the IV was already sticking 1/2 way out of her arm! Calling it a hematoma. That place is a complete dive & I feel bad for the staff and the patients! The staff is overworked & underpaid & then that trickles down to poor care of the patients! Please do not put your family here especially if you will not be up there everyday to check on them! I chose this place because of the decent reviews but was so unhappy with everything! Hopefully my review will save your loved one from having to experience this type of horrific care!
On a positive note, there was one CNA who was amazing with my mother & took great care of her. I believe her name was Thomas. Also, my mother’s occupational therapist Gail was amazing and went above and beyond! 💕

Read moreJan.2020. My mom has been a resident since June 2018, and I couldn't be more pleased. I had many reservations about nursing centers in general and those concerns were alleviated once I went to Nassau Rehabilitation & Nursing Center. Right from my initial unannounced visit I felt a friendly and caring atmosphere. During my tour I was showed the entire spotless facility. I would also like to say at this time, there was no odor anywhere. I have an extremely sensitive nose and cannot tolerate odors that I came across at other facilities. It smelled clean. That's because the custodial staff is diligently working to maintain the high quality of cleanliness. I made the decision this was where my mom would get the care that she needs. Ms. Weathers helped me though the initial paperwork and Rosa the social worker made the transition seamless. I have had almost 2 years to make observations before writing this review. I come at different times each day. The entire staff is genuinely friendly towards the patients, visitors and each other. The administrators, nurses, Certified Nursing Assistants, recreation staff, housekeeping, maintenance, custodial and security staff greet you with a smile and a friendly word. They address residents by their name. That’s huge in my book. When my mom first “moved in” she was asked a lot of questions among which was what time she liked to get up and when she liked to shower. She is given a weekly menu to fill out and food services always remembers her salad and Italian dressing. She was assigned a Certified Nursing Assistant (CNA), Antione, who is a wonderful woman. She looks after my mom as if she was her own mom. When she is not there, other CNAs look after mom. Miss Michaels, Sonia, Marsha and the CNA my mom refers to as “the singing CNA” just to name a few. Ms. Bennett, Ms. Terry and Ms. Androtti are some of the nurses who make sure mom has her medication and is doing well. Ms. Sharpe, the nursing supervisor, addresses any concerns my mom and I may have and resolves those concerns in a timely manner. Mom is well taken care of in all aspect of daily living. She is mostly wheelchair bound but is walked by Antione, so she does not get stiff. The recreation staff under Ms. Darby is friendly and goes the extra mile to make sure all residents have an opportunity to participate in the many planned activities. Michelle from recreation is assigned to my mom’s floor and does a fabulous job decorating for EVERY holiday, transporting residents and patients to activities and encouraging residents to get involved as much as their situation permits. Sean is another recreation employee for whom my mom has a soft spot. She always gets a big smile and he comes over to greet her personally. I cannot say enough about the entire recreation staff. The staff in the rehab area, OT and PT are friendly, helpful and are genuinely concerned about the welfare of their patients. John, Joe, Ms. Anderson, Mr. Chen just to name a few. When mom needs her hair done, Emily is the full-service hair stylist that comes to Nassau. She does a great job with mom cutting, coloring and styling her hair and making her feel good about herself. She now has a standing appointment every two weeks! Nassau also has a concierge. His name is Nissan. He is the “go to guy”. He can be found walking around the building, with his iPad, saying a friendly hello, taking notes or writing down concerns. Then, presto! Things get done, concerns addressed, or the problem solved. Doesn’t matter how big or small. He is the best! My review would not be complete without mentioning Cidi who is in the lobby making sure everything runs smoothly and Nick from security who knows absolutely everyone. I know I have left out the names of many wonderful employees at Nassau. My 92-year-old mom forgets sometimes. They have a sign in the lobby. I always wanted to express my sincere gratitude and to say thanks to the ANGELS god sent us.I kept putting this off as I try to get adjusted to our new way of life . This was so overdue and maybe a bit long.My mom was originally admitted to Nassau for rehabilitation and she was admitted to 2 south . The experience we had with the staff on 2 south made my decision for her to return there when she needed long term care so easy .My sincere gratitude to the team on 2 south who treated my mom with such care and always go beyond to make sure she was her best and give the emotional support when I was also having my difficult days which was many.I cannot forget miss Carmen from physical therapy that my mom looked forward to everyday .She definitely treated her with such care and she would go beyond her scope of duty to ALWAYS make her develop a sense of autonomy that physical therapy became her daily adventure. I will always be indebted to her. Her skills in working with her allowed mom to return home . Returning to home was short but when she needed long term care yes!!! I chose Nassau Rehab again. My mom was placed on 2 north and there her care takers did the UTTERMOST best for her. The team made me felt so at home . They sure made me felt I was a part of their family and sure helped me through my melt down moments when her status changed .I don’t remember all the names and some I never met since I am usually there in the day and evening time. I hv to say KUDOS!!!to all the staff on 2 north especially Miss Antoine, Terri, Miss Andrade, Sonia who were always so kind and patient and made me felt at home. Yes ,and Shondell my angel. I have to extend my sincere thanks to all but Shondell... was my mom evening Aide.I saw her a lot. I have never met someone like her . She was meant to do the job she is doing . Some people walk into our lives and leave an impact and so she has.. She is one of the BEST !! i have met. My mom is now on 4 south. I hv not met the staff because of COVID. I appreciate everyone that’s part of the staff who have participated in her care . I know it’s a tough job. Thanks for all you do. I cannot close without expressing my sincere gratitude to Yoni who took over from Nissan , and took over all the tasks that Nissan have been doing for me without a frown . Thanks Yoni for getting my questions or issues resolved and also for accommodating my schedule when dealing with my mom and especially her clothes pick up. How can I not mention Cheryl (admissions) one of my other Angel who is always ready to jump when I request her assistance or when there is no one to help me. She is always ready no matter how busy she is . She has done so much and always ready to set up my mom food for her when I drop it off and so much more ,always offer to face time . What a treat! .
