Concord Nursing and Rehabilitation Center — Local services in New York

DO NOT SEND YOUR LOVED ONE HERE.. Mr.Durant is a social worker who should be fired he should not be allowed to help patient HE DOES NOTHING.ongoing problems ever since I put my grandmother there Nurses are horrible and they have attitudes . I went to visit my grandmother once and she was slurring so bad that her dentures were falling out of her mouth when i questioned why that was happening and asked the nurse to check her sugar she told me no that they just given her her meds I demanded and asked to speak with a supervisor sure enough they checked her sugar and it was so high they had to give her another shot and then she looks and tells me it must of been something she ate , Really you know shes a diabetic why would you give her something she couldnt have. They lost her dentures and claimed she lost them(Shes bed bound) never told or contacted the family my grandmother lost 15 lbs in a month because they weren't feeding her the way they should and please keep in mind that we hired a private nurse... Also she was hospitalized for a stroke and we find out that she was having kidney failure due to severe dehydration... I called the head of the dept and have yet to receive a phone call back... Please do not send your loved ones here they will suffer!

Update my grandmother since passed away only 1 week after going back to this Hell hole from the hospital... She died on Sunday night and no-one called me to let me know until Monday afternoon when I asked why wasn't I called last night Dr.Briggs tells me well ma'am I'm calling you now... Finally I get to the nursing home and met MS.Mcfadden when I asked her why hasn't she returned my call she told me she directed it to the correct person but who did she direct it too if she didn't know why I was calling I have all the audio just in case anyone would like to hear.. I also told her that I want an autopsy and if we need to sign anything she told me and my sister no that I should have the funeral pick up her body and that they will help me with the autopsy... Sure enough when I called the funeral home and asked I was told DO NOT TOUCH THE BODY .. If we pick up the body medical examiner won't do it and I would have to get my own autopsy done out of pocket... Shouldn't the director of a nursing home know this and why would she give me an the wrong advise it's because she didn't want an autopsy done.. we went to see my grandmother's body and she(McFadden ) told us to wait because she wanted to make sure that her room was clean... Really she died last night and now you are checking sounds so dam fishy.. I have audio of all this just in case anyone would...

My grandma was at this rehab center from January to May after a leg amputation due to bad blood circulation. Since this was our first time admitting anyone from our family to rehab we just disregarded the reviews since we needed my grandma to go somewhere. This facility wasn’t that bad and most of the staff were nice but the food was nooottt good. See the photos, this was a daily reoccurrence with her meals and when speaking with the dietician he would just say she needs carbohydrates for since she needs to gain muscles. You can be the judge of the meals. I also disliked we would never get updates after medical visits, I would have to call the social worker to request the nurse manager to provide the after visit summary. Yes, they do take the patients to doctor appointments but many times they would be missed or “rescheduled by the clinic” (not true) unless we were on top of it. My grandma only speaks Spanish and there were very limited resources, only few staff members spoke Spanish and the ones that didn’t would not use interpreters they only used certain words in spanish making my grandma very frustrated. I discharged my Dad today. This nursing home and rehabilitation center may be older, but my Dad wad taken care of very well, his complaint was the food. I have friend with her mom in brand new SNF in sheepshead bay and was not happy with her care. It depends what you and family need. My family is from BED-STY, and wanted my Dad to have visitors .Do your research , no Dr. or hospital can make you go anywhere you and family don't want. I think that we must write our reviews to people who own the facility, in closing as for Mr Durant, my experience was like social worker who trys to help everyone , I have written my letter, To MRS NASH-COX. social worker , I called once, never got call back. Mr. Durant called me back, forward all necessary paperwork to agencies to have my Dad go home. This is 6 weeks , I don't want to do over,but if I did I would choose CONCORD.
