I visited this establishment in the beginning of December 2013. I ordered the sweet pork salad from an unhappy to serve or be in the building crew I paid and sat down too eat the salad two bites in I bit into a bone that sliced up my gums and crushed THREE of my teeth i went to the manager and told him about it ((refused a refund or a remake of my food)) he told me he would start to fill out a claim form he then took the bone away and put it into a container that he said he would give to his manager, about an hour later he came out of his office with the claim form I filled it out and gave it back to him the manager (Amado Martinez) said his insurance and manager would get in contact with me so I was to wait to hear from them. I didn't hear from them for 2 and a half months, after trying for a month I was able to get ahold of a lady named Liz she apologized and told me it was her fault for not calling me because SHE THOUGHT SOMEONE ELSE WAS GOING TO DO IT AND THEY WERE WAITING FOR THE BONE TO BE SENT TO THEM FROM THE MAIN MANAGER..... we got the case started and two weeks later I got some mail from costa vidas insurance company saying that the claim was started too late so they have to deny my claim. today (1/14/2014) I finally managed to get ahold of the main manager I asked him why he took so long to report this and what happened to the bone he was VERY rude he wouldn't answer ANY of my questions and he hung up on me. i guess he has better things to do than listen to ex-customers that got hurt in his restaurant. my advice to any one who reads this DO NOT EAT AT COSTA VIDA ON 501 W 2600 S. Read moreI just had my last visit to Costa Vida. I was told that a coupon card that I have is not valid (even though I have been buying and using them for over a year now), and that tonight is the very last night they are going to accept them. I have many family members and friends who have bought these Costa Vida coupon cards, and without any warning they will no longer be accepted. Also, I did not have my reward card on me and they said they were completely out. I stated that was fine because I could bring my receipt later and they could give me the points. I was told it is store policy not to reimburse the points, even if they are out of cards. Costa Vida truly does not care about being fair to their customers. I have had more than one negative experience with Costa Vida. The employees often act as thought I am inconveniencing them when I order my food. One employee accused me of lying when I told her she had not given me my receipt even though five minutes later she brought it up to me, crumbled up because she had accidentally thrown it away. And this same employee has spoken negatively about my boyfriend and I in Spanish with her coworkers as they prepare my food, thinking that we don't understand them even though my boyfriend speaks fluent Spanish. I have tried to give Costa Vida the benefit of the doubt, but I refuse to be treated the way that Costa Vida has consistently treated me. Read morePlaced an order through the Costa Vida app for the Bountiful location for a 2 taco meal. I requested no beans, no pico, no lettuce, no cheese. My husband asked before leaving with the food why there is no rice and was told rice wasn't ordered. Then we noticed there was cheese on the tacos. I returned to the store with my order. They completely ignore the fact that there is cheese is on the tacos although the order clearly states 'NO cheese'. They stand by the claim that rice was not ordered. I try to explain that the app only asks if you don't want these items. It is assumed everyone gets rice unless it says no rice. The manager then points out that the order says no beans. Not really sure what no beans has to do with no rice but it was clear nothing was going to be resolved, so I requested a refund. The manager tells me I will need to contact corporate for a refund. Never located a phone number for corporate but did find an online reporting feature. Hopefully someone will be in touch soon. This is not the first time I've had issues with the Bountiful location but it's the first time I couldn't get it resolved with the manager. I will never visit the Bountiful...
