
My mother received acute rehab at Frazier after her hemorrhagic stroke. Her stay at Frazier lasted about two weeks before being transferred to a different rehab facility. She received care on the 8th Floor of Frazier Rehab. Her Physical Therapist (Jennifer), Occupational Therapist (Debbie), and Speech Pathologist (Zaina) were all FANTASTIC. Each of her therapists put in time and effort that showed that they cared about their patient(s) as well as their job. Fortunately, my mother was also assigned several nurses that took great care of her as well. Some of her nurses while on the 8th Floor at Fraizer that really deserve a round of applause are: Spencer, Laura, Candice, Nikki, and a few others. Some of her aides were great as well; Spomenka, and Bre being two of those. Due to my mother’s stroke she went to Frazier still unable to move the left side of her body and without a skull flap on the right side of her head. Because of her inability to move her left side as well as the absence of a skull flap, my mother was assigned by PT as ONLY a hoyer lift (unless the physical therapist was present). One day my mother was in her wheel chair & she asked her aide, Yolanda, to put her back into bed. It was stated on the info board in my mother's room that she was a hoyer lift, but Yolanda said, "I'm not messing with all that." They stood my mother out of her wheelchair, make her take a few steps, and then get into bed. They also did this without my mother's helmet being on (which she needed because she didn't have a skull flap) or her shoes/non-skid socks on. So, if she'd fallen when they were transporting her against guidelines she could have been severely injured due to the fact that the only thing protecting her brain was her scalp. After this incident I spoke to the floor manager, Ronnie. Ronnie was incredibly apologetic and he took Yolanda off of my mother's service. On a Saturday, my mother had Ronnie as an aide & Mary as a nurse. I arrived to visit with my mother around 4 o'clock on the afternoon. When I arrived she was un-bathed, hadn't had her teeth brushed, and hadn't been out of bed all day long. She told me that she hit the nurses button around 2 o'clock & asked to be put into her wheelchair. When I arrived, two hours after my mother made her requests, I brushed her teeth myself & made her same requests. We were told by Mary & Ronnie that my mother would have to be cath'd at 5:45 so Mary didn't want to get her out of bed because, "once we get her up she'll just have to go right back." I insisted that she get up since she had an hour and a half before needing to be cath’d. She was not bathed until the next shift came in that evening. The next time Ronnie was her aide, Ronnie left her in wet clothes and in a wet bed for nearly two hours after an ice pack opened up on her. I spoke to the charge nurse, Brandi. Brandi came in and apologized & took Ronnie off of my mother's service. Tomeka was her aide on the day of discharge. My mother told me later that day, after being discharged that Tomeka would deny her the bedpan and wouldn’t give her a bath because, “day shift doesn’t give baths.” She also wouldn’t brush my mother’s teeth for her. Upon leaving Frazier, we (family) unpacked all of my mother's belongings. As we were unpacking we realized that some of her items were missing. We called Frazier and were given two different stories, by Rebecca, about what happened to these items. I picked up her pillow from Frazier the same day, but I spent $20 replacing the items that were missing. I am reviewing Frazier (the 8th Floor) with one star for all of the truly fantastic people that we encountered that I mentioned by name above. If it wasn't for those people I wouldn't even give Frazier a positive review at all. Unfortunately, the 8th Floor at Frazier has quite a few aides that really don't need to be working in the medical field. Read moreIf I could give zero stars I would. Let me preface this by saying my son received too notch life saving care from his doctors and nurse at UofL trauma center, top notch. He was then taken to Frazier, this is after a GSW to the neck that left him a quadriplegic with a trach and feeding tube. Paralyzed from the chest down. While in their care the only real progress he made was they taught him to feed himself, strengthened his neck and removed his feeding tube. He had a handful of good nurses and good PCAS, as well as a couple of good unit secretaries and the therapist were pretty good. Now let’s talk about cons. He lost 40 pounds while in their care, it was two weeks before he ever even saw a nutritionist there. He left with a Stage 4 pressure ulcer with osteomyelitis (this could kill him) While he was there is family was visiting staff would not do their jobs and expected family to do everything. They let him lay in his own urine and feces for hours. They did not turn him every two hours. They maybe showered him twice a week if he was lucky. He described as being treated like an animal. They would not apply his prescribed lotions to his extremities, after being there for 4 weeks I was able to scrape a cup of dead skin from his feet. He would have to wait hours for medication. He has an injured arm that is extremely painful and they never figured out why. He ended up with a uti and fungal infection in his bladder because of them straight cathing hik without proper technique. He rarely saw a wound care nurse even thought they knew he had a wound and the most of the nurses never assessed his wound properly nor did they change his dressing right. I had to do a ton of discharge planning legwork to get him home. Continuity of care is pisspoor. And the food is absolutely disgusting. Dr. Castillo gives patients “come to Jesus” threatening talks if they aren’t progressing like he wants, but he never actually does an assessment on them. When I found his wound three days prior to discharge the team including Dr. Castillo admitted it was their fault and they had made a mistake. They honestly deserve -1000 but unfortunately that's not an option to warn others to seek service elsewhere for yourself and family if you truly care about your life and/or theirs. These people are so incompetent that its pathetic! They had multiple doctors and surgeons working on my Grammy and we basically had to relay all information in between them because apparently they dont communicate with each other when working on a patient, like how unprofessional and dangerous can you get when dealing with someone's life!! To many of the nurses need to be working in a factory instead of hospital with a the nasty customer service they provide and how negligent they are of their patients not checking on them for hours even after called!! They need to tear this place down and try again with people who actually care about their patients and job!! They're also lacking in the cleaning department which is another red flag for a hospital that should be as clean as possible!
