I catered an event 6 months ago, and after picking up the order, I got a call from the store that they forgot my chips. No big deal-- turned around and got the chips. Staff were very friendly.
2 months ago, I placed a catering order again. This time, as I was setting up the buffet for the event, I came to realize that 3 of our sides were missing (cheese, beans, sour cream, and corn salsa). I called chipotle, got their support line. That person was helpful and said he would connect with the store and call me back. 30 minutes later, I got a call back from customer service saying he couldn't get ahold of the store, so he offered me a partial refund. I was grateful for the 'make it right' approach for customer service, but annoyed that the problem wasn't resolved for my event.
This week, I placed a personal order online for a couple of kids meals and a burrito bowl. All items had ordered fajita veggies; I got home, and none of them had fajita veggies. This is particularly glaring in the kids boxes, because there's just an empty section. No one offered a substitution, context for the absent ingredient, nothing. I didn't report the issue, because I'm not trying to get flagged as problem customer, and ultimately this error was annoying, but it wasn't that serious.
TWO DAYS LATER my colleague ordered from this location for a departmental working lunch. Half of the orders were missing, so someone had to leave the meeting to retrieve the remaining orders 45 minutes later. Even after that, a number of sides were not completed at all. This on its own was disruptive to the with meeting with half the people getting to hungrily watch the other half eat, but this meeting was for our primary events team, who have to coordinate catering for events regularly, and nearly EVERYONE present (15 people) started sharing stories about similar issues they had all had with the Louisiana location.

Girl at the beginning of the line (tortilla girl) waited until after my bowl had been started (rice and beans complete) to tell me there was a 5 minute wait for chicken. As I waited behind 2 other people, who were also waiting for the chicken, I noticed the manager and possibly another manager in the back playing on their phones and completely not engaged to what was going on. After waiting longer than 5 minutes, and watching how not only are the two people in front of me waiting for chicken, now the 2 people behind me are waiting for chicken, plus the 2-3 to go orders I watched pile up for chicken and still NO CHICKEN, I asked for me protein to be switched to steak. The next employee, took what is now my cold rice & beans, added the steak and additional condiments, without even acknowledging that I was obviously upset or apologizing that their was such a wait. The girl at the end of the line took my money and handed me my bag. After I was handed my bag, I requested that they remake the bowl because I had to wait, half my order was cold and nobody even acknowledged that there was a problem here. The girl who took my money plus the tortilla girl proceeded to verbally tell me that I shouldn’t request them to remake it and that I should have said something before they “finished” the bowl and that I only waited 4 minutes. Again, the 2 managers were nowhere to be seen, still in the back on their cell phones, not engaged with the customers or employees. When I asked to speak with the manager, she nor the other manager could offer any explanation, apology or even acknowledge the situation. Neither of them would provide me their names or the tortilla girls name, stating it’s company policy to not provide employees names.
