My name is King. I was a resident at this facility and their staff was HORRIBLE with the exception of the front desk and security staff , Anthony from Nutrition, Paul Williams of medical records who was very helpful although the nurses messed up my documentation, partial help from the workers in social work NOT DANIELLE, the Physical and occupational therapy staff and a few nurses namely: Ms Davis, ms Carter Ms Washington, Ms Ashley ,Ms Pierre and the CNA Ms McCloud. The LPN Cardozo was very rude, condescending and disrespectful. She wrote the wrong documentation for my test result and said I refused test that I did take as well as wrote that she gave me medication when she did not which caused me an infection and an abcess. She also said it was good that i got shot and i got what i deserved because i was in there for gun shot wound recovery and used it against me because i informed the nursing supervisor that she didn't give me my medication. MS Mahraj was extremely disrespectful, refused to give me her name when her name tag was not displayed and delayed my discharge paper work while I was in cardiac trying to get to the hospital with a heart rate of 139. Danielle from social work changed my discharge to an AMA on her own authority while i was at the hospital because she did not want to deal with the paperwork for me to be reinstated to the facility and said quote I'm not her problem. My things were packed before I returned to the facility my money and belongings were stolen while I was at the hospital by a resident who was supposed to be watched on a One to one. The staff proceeded to tell me they thought I knew her so they did not intervene. When I came home only to realize the staff had stolen my mothers tupperware and thermos that she left with me at the facility because they were not in the box they packed before my return to the facility. I was told by the janitorial supervisor they have no control over the packing the CNAs pack the box. When my father was first placed in your care, I’ll admit I was skeptical mostly because of the horror stories you hear about nursing facilities. But those worries were quickly put to rest. I was beyond comfortable leaving my father in your hands, and words can’t truly express how thankful we are for the exceptional care he received.
A special shoutout to the social workers especially Sirlina, his social worker—from day one, you reassured us that he would be okay, and you never fell short of that promise. Thank you for not just “selling a dream,” but truly delivering with passion, dedication, and heart.
To all of his nurses and aides—THANK YOU, THANK YOU! (Too many of you to name, but you know who you are.) My father isn’t the lightest man to lift or care for, but you all handled him with such skill and compassion.
To the entertainment crew, especially Allison. Thank you for always having my dad’s back, looking out for him, and even keeping him in check (lol). The entertainment at Crown Heights is unmatched, your loved one will never be bored here! From live singers, steel pan performances, special luncheons, church on Sundays, live DJs, bingo, and so much more. You all really kept my father engaged and smiling. I loved it so much that I even began volunteering myself.
The fact that residents can also enjoy haircuts, braids, and personal grooming, in addition to daily activities, shows how much you truly care for the whole person and not just their medical needs.
Lastly, Anita what a kind soul. You are a true asset to this facility. Thank you for encouraging me to volunteer and for caring for the residents with such genuine warmth.

Read moreI just left this Nursing Home from visiting my Mom on 03/28/17 between the hours of 5:30 p.m and 6:00 p.m. . When you have clothing items for your family member you inform the staff at the Front desk they give you a form to fill out listing the items it is similar to a receipt. The woman at the Front Desk was so annoyed with my mom because in her words" she shouldn't be talking to her" My husband put that woman in a place and let her know you don't have the right to blatantly disregard the residents this is supposed to be their home. When you work at facilities where the residents have certain special needs first and foremost your heart must be in it if not what's not in your heart will always come out. That woman staff member behind the desk began to stand up, raise her voice and point her finger. She was extremely unprofessional.I highly recommend this facility hire an organization that can come in and train the staff on dealing with the public and having compassion for the residents. The staff member should remember the residents there are also someone's parents. I will definitely seek other living accommodations for mom. If you can find other alternatives for your love ones do so maybe when people began to discharge their love ones the staff will have better respect. The owners should send out surveys to all the family members. Channel 7 eyewitness news should do a story on tacky unprofessional staff members who clearly forgot they too have love ones and you never know what the road ahead has in store...
