This is some of the best Sushi I have ever had. The only sushi I have had that is better was in Ginza and it costed 25 times as much money (that was in 2007 so likely much more expensive now). My girlfriend and I ate there 3 times in our 5 days in Osaka. It was our last meal before we traveled to the airport for our flight out. It was so good that I will not be able to eat sushi anywhere else until I can forget how good the sushi was here. It was full of locals and we only saw some non-locals once and only because I spoke to them while waiting in the queue and basically told them, "hey dude, in case you're wondering, yes this place is worth the wait". At most we waited 30 minutes in the queue which is no big deal. The staff also recognized us on our third visit. Perhaps because my girlfriend and I stand out. Note that the english picture menu doesn't have all items but we managed to also order some oyster sashimi from the wall menu which I think is like daily fresh items that are not always available. The restaurant is a tiny little place with mostly counter seating, but you get a chance to see them prepare your sushi for you. The service can be slow but they prepare a full order at a time (so I would recommend ordering quite a bit all at once) and will bring everything out. They do not speak much English but the menu has pictures and you just write down the menu item number(s) of what you would like to get.
The other great part is this place will not break the bank. A 3 piece of sushi ranges from ¥150-300; our bill for 4 people was only about ¥3600!!

Price was cheap and sushi was decent. Ordered 5 items for 1350 yen.
Noticed mostly Japanese people eating there.
Was confused when paying, wasn’t sure if I need to go to the counter or pay at my seats, and the only lady staff there left me standing confused and lost for a couple minutes with money.
She was prioritizing to help other customers first but I wish she could have treated me better, felt uncomfortable standing around not knowing what to do.
